MINUTES OF THE 2nd REGULAR SESSION OF THE SANGGUNIANG BARANGAY OF CAMAMBUGAN,

STA.BARBARA, ILOILO HELD AT THE BARANGAY HALL ON JANUARY 20, 2019 AT 9:00 A.M.

The meeting was called to order by the Presiding Officer at 9:00 o’clock in the morning. A
minute of prayer was led by SB Member Efren Simpao.

Since there was an attendance sheet being signed by the members as they come inside the
session hall and the presence of a quorum was determined, the roll call was dispensed with.

The minutes of the previous session dated January 6, 2019 was read. There being no
corrections offered, on motion of SB Member Suacillo and was duly seconded by SB Member De Leon,
the minutes of the previous session was adopted.

COMMITTEE REPORT:

1. SB Member Suacillio informed that tomorrow, January 21 the MIWD personnel will visit the
project and upon the arrival of the MIWD personnel Mr. Ryan Suplido of GST will be informed. SB
Member Ganat said that before the boaster pump operates; the water meter will be read (and must
have a reserve water meter in case of emergency). He also suggested that the circuit breaker will be in
padlock.

Business for the Day

1. Fun run sponsored by the PNP scheduled last December 15, 2018, was re-scheduled on
February 16, 2019 at 5:00AM. Starting line is at Robinsons Pavia.

2. There will be a giving of vitamin A, deworming and operation timbang for 0-6 years of age on
January 21, 2019 at 8:00 AM. at the Barangay Hall. Zone leaders are required to inform that Maam Janet
Samaniego, Midwife ll, BHS, Talanghauan said that BSPO Henry Sulleza didn’t attend the monthly
meeting. Maam Janet Samaniego required BSPO to attend the meeting.

3. Punong Barangay informed that Ma’am Lany Somcio provides a sample form of certificates
of indigent to every barangay. She advised that Punong Barangays will sign the certification in a blue ball
pen to distinguish the original copy to a photocopy. Two photocopies of certification will be issued per
patient.

National and Local Election - May 13, 2019
Gun Ban - January 6-January 15, 2019
Liquor Ban - May 12-13, 2019
Campaign Period(Nat’l.) - February 12-May 11, 2019
Campaign Period(Local) - March 29-May 11, 2019
Comelec Office required the barangay without or lacking SK Officials to list the 15-30 years
old SK Officials Election was not yet scheduled.

5. The MSWD required the solo parent in the barangay to fill up the application form.
Solo Parent Categories:
1. Women with child as result of rape (keeping and raising the child.)
2. Parent with sole responsibility of parenthood due to death of spouse
3. Parent with sole responsibility of parenthood due to legal separation for at least one
year.
4. Parent with sole responsibility due to imprisonment.
5. Parent with sole responsibility due to physical or mental incapacity
6. Unmarried parent who has custody to his/her child
7. Any family member who assumes the responsibility of the head of the family with
child or children as a result of death, abandonment
8. Any person who solely provides parental care and support to a child or children
Requirements:
1. Birth certificate of the dependent minor children
2. Barangay certification that the applicant is a solo parent
3. Certification of employment if applicant is employed
4. 1 x 1 Picture (2Pieces)
Application forms will be submitted to the MSWD Office until February 28, 2019

Other Matters:
1. Benefits of Barangay Officials – from LIGA
Medical Assistance:
Punong Barangay (admit hospital) - 3,000.00
Barangay Officials (admit hospital) - 1,000.00

2. There will be a summit on January 22, 2019, at the Central Philippine University to be
attended by the Punong Barangay.

There being no more topics to be discussed, on motion of SK Chairman, Eden Mae Gorantes
and duly seconded by SB Member Efren Simpao, the meeting was adjourned at 11:00 AM.
