3 SUBMISSION OF APPLICATION FORM

Application form to be submitted through on-line method in two steps:

l Select Registration type Fresh or Repeater and enter RTMC Number.

STEP-1 l Confirm your email address and submit.
l A verification code will be sent to your email address.
l Enter the verification code for further proceeding.
l Complete the application form.
l A PDF format of your form will be generated and sent to your email address.
l Take a print of application form.

After completing step-1, please process to step-2.

Before proceeding step-2 ensure that you have converted your all original documents in
STEP-2
PDF format and paid your fee.
l Enter your form number and verification code.
l Upload all required documents in PDF format and submit.

l Original Bank draft or pay order must be submitted to CPSP Regional Office or finance department
CPSP Karachi.
l For further correspondence please mention your name, cpsp id, subject and Examination centre
from where you are appearing in the examination.
l PLEASE NOTE THAT SEPARATE APPLICATION FORMS ARE TO BE USED BY FRESH APPLICANT
& REPEATERS.

5 REFUND OF FEE
Seventy five percent of the examination fee shall be refunded to the candidate after deduction of 25% fee
for processing,subject to:
l Receipt of application for withdrawal from the examination if received on or before the specified date.
l Rejection of the application by CPSP on account of ineligibility.
While applying for REFUND, candidate should write his / her NAME, CENTRE, SUBJEC T and BANK
RECEIPT NUMBER. 9
In all other cases the fee paid for a particular examination will neither be refunded nor carried over
to next examination in case of absence / exclusion from the examination for any reason,what so ever. .

CANDIDATES WHO QUALIFY IN THEORY EXAMINATION AND TOACS COMPONENTS CAN APPEAR IN THREE OUT OF
FOUR CONSECUTIVE CLINICAL EXAMINATIONS (INCLUDING MISSED CHANCES) WITHOUT APPEARING IN THEORY
EXAMINATION AND TOACS COMPONENTS AGAIN. HOWEVER, A CANDIDATE WHO DOES NOT APPEAR IN
TOACS/CLINICAL EXAMINATION OF THE SAME SESSION AFTER PASSING THEORY EXAMINATION, WILL LOSE ONE
CHANCE . ONLY TWO OUT OF THE NEXT THREE CONSECUTIVE CHANCES WILL BE AVAILABLE TO SUCH CANDIDATES.

8 VENUE OF EXAM INATION

THEORY: The Online Theory Examinations are conducted at CPSP Karachi, Regional Centre Hyderabad,
Quetta, Multan, Lahore, Faisalbad,Islamabad & Peshawar.
The choice of the city should be clearly written in the appropriate box in the application form.
The CPSP reserves the right to delete a particular centre, or shift the candidates from one Centre to another
on account of logistic reasons.
ORAL/CLINICAL: The College will hold clinical examination ONLY in one or more cities of PAKISTAN
depending upon the number of candidates eligible to take this examination and logistic facilities available in
that city irrespective of the venue at which a candidate appeared in theory examination. Exact Venue of
Clinical examination will be communicated after the assessment of theory examination papers.

9 TRAINING COMPLETION CERTIFICATE

l Training must have been completed one month before the date of theory Examination.
l Experience certificate should be on letterhead of institution, signed and stamped by both your RTMC registered
Supervisor and Head of Institution.
l Completion of four years training (from date of commencement of training to date of completion of training)with all
rotations is a mandatory requirement to appear in FCPS-II examination. No relaxation in training duration is permissible.

10 E-LOG ENTRIES
Entries in elog book should be made timely on daily basis during the course of training and get it approved by your
Supervisore. Back dated (more than 3 months old) entries in elog book are not allowed.

NOTE : ONLY SCAN COPY OF THE ORIGINAL DOCUMENTS IS ACCEPTABLE. SCANNING OF PHOTO COPIES
