AFFIDAVIT OF CONSENT

I, ANNA LIZA R. NIPA, of legal age, Filipino and presently residing at lot 10, C Lawis Ext.
Sitio Dalisay Brgy, San Isidro, Antipolo City, after having been to in accordance with the law
depose and state, THAT;

1. I am the owner of a Commercial Building located at lot 10, E Rodriguez Ave., Brgy. San
Isidro, Antipolo City;
2. That I am Following WENDY SIXTO BRIGAS to use and occupy the commercial building
for free of rental and without his paying anything for its occupation and use;
3. By virtue, hereof, I am giving consent to WENDY SIXTO BRIGAS for his to use said
premises for the sole purpose of the operation of his business Permit Only;
4. I am likewise executing this affidavit, in order to attest to the truth and for whatever lawful
purpose to this may serve
