AFFIDAVIT.

I, LOVELINA F. LIM, of legal age, married, Filipino citizen, and with residence
and postal address at B 17 L 28 DECA HOMES 1, AGUS, LAPU-LAPU CITY,
CEBUafter having duly sworn in accordance with the law, hereby depose and state that:

1. That, I am the wife and Attorney-in-fact of JAMES C. LIM with special

power of attorney executed on February 01, 2020, with Doc. No. 152,
Page No. 32 Book No. XXI Series of 2020 in Cebu City under the notary
public of Atty. Edward R. Bauzon.

2. That, the signatures reflected on the loan documents were inconsistent vs

my valid ID’s

3. That, I can no longer perfect my signature as reflected in my valid ID

despite the diligent effort.

4. That, I am now using this signature as my valid and personal signature

which was also reflected on all loan documents on Mr. Garlet's housing
loan.

5. It is with this purpose that I acknowledge the signature appearing on the

application thru Pag-IBIG Fund are my original signature despite of its
inconsistency
