ERECTILE DYSFUNCTION - IMPOTENCE

Repeated inability to get or keep an erection firm enough for sexual


intercourse.
Erectile dysfunction, or ED can be:
a total inability to achieve erection
an inconsistent ability to do so
or a tendency to sustain only brief
erections.

• Therefore definition of ED and estimation of its incidence is difficult.

• Estimates range from 15 million to 30 million men (in the USA).

• According to the National Ambulatory Medical Care Survey (NAMCS), for


every 1,000 men in the United States, 7.7 physician office visits were made
for ED in 1985. By 1999, that rate had nearly tripled to 22.3.
• In older men, ED usually has a physical cause, such as disease, injury, or
side effects of drugs.

• Any disorder that injures the nerves or impairs blood flow in the penis has
the potential to cause ED.

• The incidence of erectile dysfunction increases with age: about 5 percent of


40-year-old men and between 15 and 25 percent of 65-year-old men
experience ED.

• However, ED is not an inevitable part of aging.


• Two chambers called the corpora cavernosa run the length of the organ.

• The corpora cavernosa are surrounded by the tunica albuginea.

• A spongy tissue fills the chambers and contains smooth muscles, fibrous
tissues, spaces, veins, and arteries.

• The urethra, which is the channel for urine and ejaculate, runs along the
underside of the corpora cavernosa and is surrounded by the corpus
spongiosum.

The corpora cavernosa, which can be likened to hydraulic
cylinders, create structural rigidity.

The corpora cavernosa are cigar-shaped tubes of strong


connective tissue, filled with spongy muscle-lined
cavities.

Relaxation of vascular muscle causes inflow of blood and


expansion of the corpora’s inner sponge during erection.

As a result its wall, the tunica albuginea, is stretched


tightly enough to produce rigidity. In addition to defining
the shape of the erect penis, the tunica also has a key role
in limiting blood outflow during erection.

The corpus spongiosum contains the urethra, or excretory


channel.
Erection begins with sensory or mental stimulation, or both.

Impulses from the brain and local nerves cause the muscles of the corpora
cavernosa to relax, allowing blood to flow in and fill the spaces. The blood
creates pressure in the corpora cavernosa, making the penis expand. The
tunica albuginea helps trap the blood in the corpora cavernosa, thereby
sustaining erection. When muscles in the penis contract to stop the inflow of
blood and open outflow channels, erection is reversed.

An erection requires a precise sequence of events:


nerve impulses in the brain, spinal column, and area around the penis,
response in muscles, fibrous tissues, veins, and arteries in and near the
corpora cavernosa.
Damage to nerves, arteries, smooth muscles, and fibrous tissues is the most
common cause of ED.

Diseases—such as diabetes, kidney disease, chronic alcoholism, multiple


sclerosis, atherosclerosis, vascular disease, and neurologic disease—account
for about 70% of ED cases. Between 35 and 50 percent of men with diabetes
experience ED.

The same life habits that contribute to heart disease and vascular problems
also raise the risk of erectile dysfunction. Smoking, being overweight, and
avoiding exercise are possible causes of ED.
Surgery (especially radical prostate and bladder surgery for cancer) can injure
nerves and arteries near the penis, causing ED.

Injury to the penis, spinal cord, prostate, bladder, and pelvis can lead to ED
by harming nerves, smooth muscles, arteries, and fibrous tissues of the
corpora cavernosa.

Many common medicines - blood pressure drugs, antihistamines,


antidepressants, tranquillizers, appetite suppressants, and cimetidine—can
produce ED as a side effect.

Psychological factors such as stress, anxiety, guilt, depression, low self-


esteem, and fear of sexual failure may cause 10 to 20 percent of ED cases.
Other possible causes are smoking, which affects blood flow in veins and
arteries, and hormonal abnormalities, such as not enough testosterone.
ERECTILE DYSFUNCTION IN CHINESE MEDICINE

Erectile dysfunction is called Yang Wei [阳 痿] in Chinese medicine.

Yang [阳] means “Yang”


[疒] is the radical indicating a “disease”
Wei [委] means “listless” or “withering”

Therefore, the term Yang Wei indicates a disease characterized by


“listlessness of Yang”. Interestingly, the ancient term for erectile
dysfunction was actually Yin Wei in which Yin refers to “Yin” as in “Yin-
Yang”. This difference in terminology is probably due to the fact that Yin in
Yin Wei refers to the penis that is called Yin Jing [阴 茎] in Chinese,
which literally means “Yin Stem”.

In some passages, erectile dysfunction is also called Jin Wei 筋 痿


literally “withering of Jin” in which “Jin” stands for Zong Jin 宗 筋,
i.e. the “Ancestral Muscle” (penis).
In ancient times, erectile dysfunction was also sometimes called Jin
Wei [筋 痿] which means “listlessness of the sinews”. This name is
probably related to the term Zong Jin [宗 筋] (Ancestral Muscle or
Sinew) which was another term for the penis.

Su Wen chapter 44: “Avarice, disappointment, excessive desire and


excessive sexual activity cause weakness of the sinews leading to Jin
Wei.”

Chapter 70 of the “Su Wen” mentions erectile dysfunction (calling it


Yin Wei): “When the energy of Tai Yin predominates... … there is
erectile dysfunction from great decline of Qi with inability to rise.”
Chapter 44 of the “Su Wen”:
“When one is affected by desire without satisfaction and pursues outside
excessive goals, or when one engages in excessive sexual activity, the penis
[Zong Jin, Ancestral Muscle] will become slack causing erectile dysfunction
[Jin Wei] and seminal emissions.”

Chapter 5 of the “Su Wen”:


“At 60, there is impotence, a great deficiency of Qi and the orifices are
obstructed.”

Chapter 13 of the “Ling Shu” (on the Muscle channels):


“When the Muscle channel of the Jue Yin of the Leg [Liver] is diseased...the
sexual organ cannot be used.”

The “Complete Book of Jing Yue” (Jing Yue Quan Shu, 1624) relates the
aetiology of erectile dysfunction to emotional stress:

“Worry and pensiveness deplete [Qi], excessive worry leads to impotence.”


The “Complete Book of Jing Yue” (Jing Yue Quan Shu, 1624) relates the
aetiology of erectile dysfunction to emotional stress:
“Impotence in men is due to decline of Ming Men and Jing deficiency. Or it
may be due to overtaxing by the seven emotions and decline of Yang Qi.
Damp-Heat may infuse downwards so that the Ancestral Muscle [Zong Jin]
becomes slack and withers.”

The “Secret Formulae of the Border Official” (Wai Tai Mi Yao, 752 AD)
clearly relates erectile dysfunction to the Kidneys:

“The Kidneys control the lower orifices, overexertion injures the Kidneys, the
deficient Kidneys cannot control the lower orifices and erectile dysfunction
results.”

The Discussion of the Origin of Symptoms in Diseases (Zhu Bing


Yuan Hou Lun. AD 610) clearly related erectile dysfunction to the
Kidneys:
“The genitals are an orifice of the Kidneys, when these are injured by
overwork, the deficient Kidneys cannot nourish the genitals and erectile
dysfunction results.”
The Treatment according to Pattern Categories (Lei Zheng Zhi Cai) by
Lin Pei Qin (1839) says:
“Impotence [Yang Wei] is due loss of Jing from excessive [sexual]
desire, worry and pensiveness injuring the Shen, Damp-Heat infusing
downwards, the Ancestral Muscle becomes slack and impotence
results.”

The Ming Yi Za Zhu (1549):


“Impotence is due to decline of Ming Men and Jing Qi deficiency.”

The Mirror of Medicine Yi Jing (1641):


“In impotence, old men experience decline of Jing, young men loss of
will power [shi zhi 失 志]. Impotence in young men is due to
mental-emotional dissatisfaction [zhi yi bu sui 志 意 不 遂]
leading to depression so that Yang Qi withers.”
The Guide to Clinical Syndromes (Lin Zheng Zhi Nan, 1766)
says:
“When Yang Ming is deficient the Ancestral Muscle becomes
slack. The Stomach is the Sea of Food, if food intake is not
optimal Jing Qi becomes deficient. Men’s genitals are also
called ‘power’ [shi 势]. When Gu Qi is deficient, ‘power’
cannot harden and be raised.”

The Record of Pattern Identification (Bian Zhen Lu,1687):


“” Impotence is due to decline of Ming Men, how can it be due to
Heart-Qi deficiency? The Heart is Emperor Fire and the Minister
Fire follows it. Therefore in treating impotence one must tonify
both the Heart and Kidneys, when the Heart is calm the Kidneys
move.
The “Secret Record of the Brocade Bag” (Jin Nang Mi Lu Qing
dynasty),
“Impotence in young men is due to loss of will power [shi zhi 失
志], this causes stagnation so that it cannot tonify Yang. If there is
mental-emotional dissatisfaction [zhi yi bu sui 志 意 不 遂],
Yang Qi cannot flow freely and depression ensues so that it causes
impotence.”

“Worry, pensiveness, fear and fright injure Spleen and Kidneys,


Central Qi [Zhong Qi] is weakened and impotence results.”
FACTORS AFFECTING ERECTION

Du Mai – Kidney-Yang – Minister Fire – Fire of Ming Men

Blood – Heart-Blood

Liver channel (Main, Luo, Muscle and Divergent) wraps around penis

Pathogenic factors such as Dampness, Blood stasis, stasis of Jing.

The Chong Mai influences erection in seven ways:

1) As the Sea of Blood

2) It is connected to the Post-Natal Qi

3) It is connected to the Pre-Natal Qi

4) It controls the Membranes (Huang)

5) It controls the Ancestral Muscle (Zong Jin)

6) It controls the Luo Mai in the penis

7) It allows communication between Heart and Kidneys


Erection Qi is needed for the penis to come up
depends on both
Qi and Blood. Blood because the corpus cavernosum needs
to fill with blood

Therefore, the Chong Mai can be used to treat erectile dysfunction.

For disharmony between Heart and Kidneys with Heart-Qi and


Heart-Blood not descending to the penis, I use:

opening and coupled points of the Chong Mai (SP-4 Gongsun on


the left and P-6 Neiguan on the right) and KI-12 Dahe, Ren-4
Guanyuan, Ren-15 Jiuwei, Ren-6 Qihai, BL-15 Xinshu and BL-23
Shenshu.

BL-15 tonifies and invigorates Blood (as the Heart governs Blood)
and promotes the descending of Heart-Blood down to the penis.

KIDNEYS
Du Mai – Kidney-Yang – Minister Fire – Fire of Ming Men

• Erection needs movement and action, which are Yang characteristics.


• Erection therefore needs Yang Qi deriving from Kidney-Yang the Fire of the
Gate of Life.
• A deficiency of Kidney-Yang and of the Minister Fire is a common cause of
erectile dysfunction in both young and old men.

Blood – Heart-Blood
The penis needs the action of the Heart for erection in two ways:
1) Heart-Qi must descend to the penis to promote erection in the same way
as Heart-Qi descends to the Uterus to promote menstruation.
2) The Heart governs Blood and the penis needs to fill with blood to achieve
an erection.

Liver channel (Main, Luo, Muscle and Divergent) wraps around penis

• It has an important influence on the penis as all its channels (Main, Luo,
Muscle and Divergent) wrap around the penis.
• Therefore, the Liver channel plays a role in the pathology of erection, for
example when it suffers from Qi stagnation or Dampness.
The Chong Mai influences erection in seven ways:

1) As the Sea of Blood it provides Blood to the penis (in this sense this
function of the Chong Mai occurs in synergy with that of the Heart).

2) It is the Sea of Channels and is closely connected to Post-Natal Qi of the


Stomach through ST-30 Qichong. Therefore the Chong Mai also provides the
Qi necessary for erection to occur.

3) It is also connected to the Pre-Natal Qi of the Kidneys. For erection to


occur, both Post-Natal Qi and Pre-Natal Qi are needed.

4) It controls the Membranes (Huang), most of the penis is part of the


Membranes.

5) It influences the Zong Jin (Ancestral Muscle) which is the penis.

6) It controls the Luo Mai in the penis which allow it to be filled with blood
when erect.
7) It assists the communication between Heart and Kidneys and therefore the
descending of Heart-Blood to the penis.
AETIOLOGY OF ERECTILE DYSFUNCTION IN CHINESE
MEDICINE

Chinese books generally discuss the following aetiological factors for


impotence:

a) Old age

b) Excessive sexual activity

c) Irregular diet

d) Emotional stress

e) Excessive physical activity

f) Overwork
a) OLD AGE

As a man gets older, the Kidney-Jing naturally declines and the Ming Men
gradually diminishes. It is in particular the decline of the Ming Men that
causes erectile dysfunction in the elderly.

b) EXCESSIVE SEXUAL ACTIVITY

It causes a decline of both the Kidney-Jing and of the Ming Men: the decline
of the latter may cause impotence.

Modern Chinese books always make a point of mentioning that excessive


masturbation also constitutes excessive sexual activity.

Whether or not sexual activity is “excessive” depends on the man’s age. A


very rough guideline is provided by dividing a man’s age by 5 to indicate the
frequency of sexual activity. For example, for a 40-year-old man the ideal
frequency would be once every 8 days (40 : 5 = 8). This is only if the man is
in good health: if he suffers from a Kidney deficiency, frequency should be
reduced.

Excessive frequency of ejaculation weakens the Kidneys; sexual activity


without ejaculation does not.
c) IRREGULAR DIET

Excessive consumption of greasy, fried food and of dairy foods may lead to
Dampness or Phlegm, both of which may affect the genital system and give
rise to impotence.

d) EMOTIONAL STRESS

Emotional stress is an important cause of erectile dysfunction in the patients


we see in Western countries.
All emotions may lead to Qi stagnation, starting a pathological process that
later involves some Heat and frequently Dampness or Phlegm.
Worry, grief and sadness particularly affect the Spleen and Heart and may
lead to erectile dysfunction from Heart-Blood deficiency.
Fear and anxiety affect the Kidneys and may lead to erectile dysfunction from
Kidney deficiency.

e) EXCESSIVE PHYSICAL ACTIVITY

Excessive physical activity (which may include excessive running or weight-


lifting in gym work) weakens Spleen- and Kidney-Yang and may lead to
erectile dysfunction from Kidney deficiency.
f) OVERWORK

Overwork is another very important cause of erectile dysfunction in


Western patients.

“Overwork” means working long hours without any rest for many years,
usually also under conditions of stress.

This weakens the Kidneys and particularly Kidney-Yin and is an important


cause of erectile dysfunction in middle-aged men.
PATHOLOGY OF ERECTILE DYSFUNCTION IN CHINESE
MEDICINE

It is important to differentiate Full from Empty conditions and the main


pathogenic factors involved.

In China, primary importance is always given to a Kidney deficiency (and
particularly Kidney-Yang) as a cause of impotence.
In young and middle-aged men, a Heart pathology is a more common cause
of impotence.
This is due to emotional stress causing Heart-Qi stagnation or Heart-Blood
deficiency so that Heart-Qi and Heart-Blood fail to descend to the penis and
Heart-Blood fails to fill the penis with blood for erection to occur.

In Full conditions pathogenic factors obstructing the process that


leads to an erection.

Dampness and Phlegm (usually deriving from irregular diet)


obstruct the Luo Mai of the penis and prevent erection.

Blood and Jing stasis (usually deriving from emotional stress and
overwork) also obstruct the Luo Mai of the penis and in particular
the Blood (Deep) Luo Mai, making erection difficult.

Dampness affecting the penis usually stems from the Spleen, Kidney
and Liver channels.
In men stasis of Jing is the equivalent of Blood stasis in women. The clinical
manifestations are:

Stabbing pain in the lumbar region


Pain in perineum
Hypogastric pain
Pain in testis and/or penis
Impotence
Premature ejaculation
Priapism
Prostatic hypertrophy
Premature greying of hair
Itching or pain in the pubic region
Abnormal sperm
Peyronie’s disease
Purple tongue
Choppy or Firm pulse.
Phlegm is a common pathology of men’s genital system in the same way that
it is for the Uterus. In men, the main genital conditions manifesting with
Phlegm are as follows:

Priapism
Sweaty genitals
Prostatic hypertrophy
Impotence
Peyronie’s disease
Slippery pulse
Swollen tongue

The 6 Extra Fu are related to the Ming Men and therefore erectile
dysfunction.

The Gall Bladder controls the ascending of Qi and it influences


erection. The Blood Vessels influence the genitals because they
blood vessels wrap and bind around the male genitals. The Brain,
Marrow and Bones are all derived from Kidney-Essence and they
all influence the sexual function.
SEA OF MARROW

Excess: Full of vigour, great physical strength.


Deficiency: Dizziness, tinnitus, weak legs, blurred vision, desire to lie down.
Points: DU-20 (upper), DU-16 (lower).

The “vigour” or fullness of the Sea of Marrow includes sexual vigour;


therefore deficiency of the Sea of Marrow also includes erectile dysfunction.
The Qi of the 5 Zang and 6 Fu ascends to and nourishes the Brain. Therefore
a deficiency of the Zangfu (from emotional stress and overwork) weakens the
Marrow and Brain and this affects the sexual function.

kidneys where the Dong Qi throbs,
it goes to the uterus and the pubic

In women, it flows around the


vagina, passes to the perineum, then

THE TRIPLE BURNER AND ERECTILE
DYSFUNCTION
Nan Jing, chapter 66: “Below the umbilicus the Motive Force (Dong Qi)
throbs: this is the source of life.”

It is the root of the 12 channels and that is why it is called the Yuan.
The Triple Burner allows the Yuan Qi to separate into its different
manifestations [roles], it controls the movement of the three Qi [Yuan,
Ying and Wei] and it spreads to the 5 Zang and 6 Fu.”

The Food Essences of the Stomach and Spleen enter the Internal
Organs and they enter the Brain upwards and irrigate the Ancestral
Muscle (Zong Jin) downwards.

ST-SP
Therefore, if the Triple Burners Qi transformation is normal, the
Food Essences are spread normally, the Water Passages are open,
the Yuan Qi spreads to the 12 channels normally, the Essence of the
Zangfu is abundant, the Sea of Marrow is full, the Ancestral Muscle
is irrigated and the sexual function is normal.

If the Upper Burner is obstructed, Lung-Qi cannot


descend and the Ancestral Muscle cannot function
properly.
If the Middle Burner is obstructed, Dampness
accumulates and the Ancestral Muscle is too relaxed
(flaccid).
If the Lower Burner is obstructed, Liver-Qi stagnates,
Kidney-Qi is deficient, the urinary and genital orifices are
not open, the Ancestral Muscle cannot extend and the
sexual function is compromised.

When the Triple Burner is obstructed, the tongue coating is thick.


The treatment principle is to dissolve accumulation, eliminate
stagnation, regulate the Qi Mechanism, so that the Three Burners are
free, the Ancestral Muscle is functional and the sexual function is
normal.

Premature ejaculation may be due to:

Damp-Heat and Heat injuring Yin.

Dr Fang says that erectile dysfunction and premature ejaculation are


due to Yin Xu more than Yang Xu. He also says that:

DAMP-HEAT AND ERECTILE DYSFUNCTION by Dr Zhao Jin Ze

Most doctors attribute erectile dysfunction to a Kidney deficiency but Damp-


Heat plays an important and frequent part in it. Chapter 3 Sheng Qi Tong
Tian Lun 生 气 通 天 论 of the “Su Wen” says:

“When Damp-Heat accumulates, the large


sinews are shortened resulting in
“restriction” and the small sinews are
slackened resulting in impotence.”

The penis is influenced by the Liver channel and it is the place where the
Ancestral Muscle gathers: when Damp-Heat affects it, the Ancestral Muscle
becomes flaccid and erectile dysfunction results. Sinews and blood vessels
are involved in the pathology of erectile dysfunction as the Liver controls the
sinews and blood vessels wrap around the genitals.

The treatment principle is to move Liver-Qi, resolve Dampness and clear


Heat.
Use variations of Long Dan Xie Gan Tang plus Da Huang.
12 METHODS OF TREATING JING FOR ERECTILE
DYSFUNCTION

1. Tonify Qi and nourish Jing. Used for erectile dysfunction or premature


ejaculation against a background of Qi-Blood Xu. Under such
conditions, premature ejaculation is called “Liu Jing”, i.e, slippery Jing.
Decreased sexual desire, frequent urination, cold genitals, cold sweating
after sexual intercourse, dilute sperm. Ren Shen, Huang Qi, Dang Shen,
Tai Zi Shen, Shan Yao, Bai Zhu, Fu Zi, Lu Jiao Shuang, Sang Piao Xiao,
Ba Ji Tian.

2. Nourish Blood and Jing. Dang Gui, Shu Di Huang, He Shou Wu, Bai Shao,
E Jiao, Lu Jiao Jiao, Gou Qi Zi, Zi He Che, Ji Xue Teng.

3. Tonify the Kidneys and nourish Jing. Fu Zi, Rou Gui, Lu Rong, Ba Ji
Tian, Xian Mao, Yin Yang Huo, Sha Yuan Zi, Lai Fu Zi, Zi He Che, Hai
Ma, Suo Yang, Rou Cong Rong.
4. Restrain Qi (lift) and astringe Jing. Used when Yuan Qi is weak and there is
leakage of sperm such as nocturnal emissions or premature ejaculation.

with Acupuncture”.

100 cases of erectile dysfunction were treated with acupuncture. The age of
the patients ranged from 24 to 53.

The patients were treated once every 2-3 days for a course of 10 treatments.
After a rest of 5-7 days, a second course was given, up to 5 courses.

The main points used were: BL-32 Ciliao, Ren-2 Qugu, LIV-11 Yinlian, LIV-
1 Dadun. Additional points according to symptom were:
- Insomnia: Du-20 Baihui, P-6 Neiguan.
- Tiredness: ST-36 Zusanli.

Erectile dysfunction due to injury of the penis is distinguished from that due
to functional imbalance. “Structure depends on the sinews, function depends
on Qi and Blood”. “Sinews” are mentioned because in ancient times erectile
dysfunction was also called Jin Wei, i.e. “Flaccidity of the Sinews”.

Functional erectile dysfunction depends primarily on the Kidneys and Liver


and specifically Kidney deficiency or Liver stagnation. “Liver stagnation” is
used in a broad sense to include Dampness, Phlegm and stasis of Essence (all
of which involve the Liver channels in the penis).

Xie Pin Qi “Clinical Research on 40 Cases of Erectile Dysfunction with the


Method of Tonifying the Kidneys and Moving Liver-Qi”.

40 cases were treated with the method of tonifying the Kidneys and moving
Liver-Qi. Age ranged from 32 to 68, and from 32 to 65 in the control group.

The treatment group was treated with Bu Shen Shu Gan Wan Tonifying the
Kidneys and Soothing the Liver Pill as follows:

Henan Province Kaifeng City Hospital of Chinese Medicine “A Case of


Erectile Dysfunction treated with moxibustion”.

A 40-year-old man suffering from erectile dysfunction was treated with


moxibustion. He had tried several herbal treatments without success. His
clinical manifestations were: impotence, tiredness, lower backache, feeling
cold, dizziness, tinnitus, Pale tongue.

He was treated with moxibustion on Ren-6 Qihai, Ren-4 Guanyuan and SP-6
Sanyinjiao. Moxibustion was applied with a moxa stick for 10 minutes per
treatment, once a day for 3 months. After 3 months, he was completely
cured.

Tao Huan “The Experience of Dr Liu Yong Nian in the Treatment of Erectile
Dysfunction by Soothing the Liver and Eliminating Stagnation”.

Dr Tao Huan reports the experience of Dr Liu Yong Nian in the treatment of
erectile dysfunction. Dr Liu believes that the main cause of erectile
dysfunction in young men is not a Kidney deficiency but Liver-Qi stagnation
deriving from emotional stress. He uses a formula called Jiu Xiang Shu Gan
Tang Aspongopus Soothing the Liver Decoction. Its emperor ingredient is an
insect called Jiu Xiang Chong Aspongopus.

PREMATURE EJACULATION

The definition of premature ejaculation is highly subjective and there is


indeed no recognized standard definition of it.

Masters and Johnson define premature ejaculation (PE) as the condition in


which a man ejaculates before his sex partner achieves orgasm, in more
than fifty percent of their sexual encounters. If we accepted this
definition, then the majority of men would probably suffer from PE: so
this definition is probably too broad.
Others have defined premature ejaculation as occurring if the
man ejaculates within two minutes of penetration. Self
reported surveys report up to 75% of men ejaculate within 10
minutes of penetration. Today, most sex therapists understand
premature ejaculation as occurring when a lack of ejaculatory
control interferes with sexual or emotional well-being in one
or both partners.

Most men experience premature ejaculation at least once in


their lives. PE affects 25%-40% of men in the United States.
There is great variability in both how long it takes men to
ejaculate and how long both partners want sex to last.
The primary sign of premature ejaculation is ejaculation that
occurs before both partners wish, causing concern or distress.
However, the problem may occur in all sexual situations, even
including during masturbation.

Premature ejaculation is usually classified as either lifelong


(primary) or acquired (secondary).

According to the International Society for Sexual Medicine,


lifelong premature ejaculation is characterized by:
- Ejaculation that always or nearly always occurs within
one minute or less of vaginal penetration
- The inability to delay ejaculation on all or nearly all vaginal
penetrations
- Negative personal consequences, such as distress, frustration
or the avoidance of sexual intimacy
The International Society for Sexual Medicine has concluded that there
are not yet enough published data about acquired (secondary)
premature ejaculation to publish an evidence-based definition of this
condition.

According to the definition in the Diagnostic and Statistical Manual of


Mental Disorders (DSM), published by the American Psychiatric
Association, secondary premature ejaculation:

- Is marked by persistent or recurrent ejaculation with minimal


stimulation before, on, or shortly after penetration, and before
one wishes it
- Causes personal distress or relationship problems
- Develops after one had previous, satisfying sexual relationships
without ejaculatory problems
PREMATURE EJCULATION IN CHINESE MEDICINE
In Chinese medicine, premature ejaculation is seen as a deficiency
of the Kidneys not holding onto sperm: it is therefore seen as a
“leaking” pathology like, for example, uterine bleeding from Qi
deficiency, sweating from Lung-Qi deficiency or nocturnal
sweating from Yin deficiency. It follows therefore, that most cases
of premature ejaculation are due to some form of Kidney
deficiency.

However, the Heart also plays a role in that Heart-Qi and Heart-
Blood need to descend to the penis to promote normal erection and
ejaculation. Thus, premature ejaculation may be due to Heart-Heat
causing Heart-Qi and Heart-Blood to descend too early, thus
causing premature ejaculation.

LOW SPERM COUNT

Low sperm count means that the semen during an orgasm


contains fewer sperm than normal. Low sperm count is also
called oligospermia. The sperm count is considered lower than
normal if there are fewer than 20 million sperm per milliliter of
semen.

A low sperm count decreases the odds that one sperm will
fertilize the woman's egg, resulting in pregnancy. Nonetheless,
many men who have a low sperm count are still able to father a
child.

A low sperm count is the leading cause of male infertility.


Others may be low sperm motility, bad quality sperm, lack of
semen. In general, most cases of male infertility are due to low
sperm count. There are many biologic and environmental factors

No matter what sexual symptoms there are (e.g. impotence, premature


ejaculation, etc.), sperm count or motility problems are always due to the
Kidneys. Must distinguish Yin from Yang Xu; however, when tonifying
Kidney-Yang one must also remember Kidney-Yin and when nourishing
Kidney-Yin one must remember Kidney-Yang.

For low sperm count, nourish Kidney-Yin with Zuo Gui Wan or Zuo
Gui Yin; if there is Empty Heat, use Zhi Bo Di Huang Wan.

For low sperm motility, tonify Kidney-Yang with


You Gui Wan or You Gui Yin plus Ren Shen and
