In caring a person whether it is a baby, a child, elderly or any human who needs
care, the first thing to do is taking its vital signs and it is your duty to inform your client
about their vital signs.
This part gives will gives you a chance to be familiar of the two significant
measurements. As learning goes you will be able to learn the important of taking
exact reading in taking vital signs. But what is vital signs? Vital signs are physical
functions that replicate the body’s state of health and are easily measurable: body
temperature, pulse rate, respiratory rate, and blood pressure. In other situation, the
fifth vital sign is measured to be the pain that a person experience.
Thermometer
Body temperature is an amount of the total heat in the body. The balance
amongst heat produced and heat lost is the body temperature. The standard adult
body temperature is 37 degrees Celsius. There is an ordinary range in which a
person’s body temperature may vary and still be measured normal. Take a look at
these normal ranges of body temperature:

1. Wash your hands and take away the thermometer out of its container.
2. Clean the probe (pointed end) of the thermometer by rubbing alcohol / soap
and then

thermometer has no probe cover, start using the thermometer.

5. Put the thermometer beneath the client’s tongue on one side of his/her
mouth. Tell him/her to close his/her lips. (Though hold the thermometer
particularly if the client is sick and too weak to hold the thermometer with his/her
lips.)
6. Leave the thermometer in place till the thermometer signs it is done. When it
beeps, it indicates that it can be detached.
7. Take away the thermometer carefully and read the temperature on the
numerical display. Clean the tip of the thermometer with a cotton ball dipped in
alcohol. Place the thermometer’s tip cover. Put the thermometer in its
container.

4. Put the thermometer under the client’s armpit. (You can hold the
thermometer particularly if your client is too sick and frail that he/she cannot
even grip the thermometer with his/her armpit.)
5. Leave the thermometer in place until the thermometer signs it is done. When
the thermometer beeps, it means that it can be detached.
6. Remove the thermometer slowly and read the temperature on the numerical
display. Clean the tip of the thermometer with a cotton ball dipped in alcohol.
Place the thermometer’s tip cover. Put the thermometer in its container.
BP Apparatus
One more vital measurement that you must study is to take is the blood pressure.
Blood pressure is the force of the blood push against the walls of the blood vessels.
The heart contracts as it pushes the blood into the arteries. When the heart is
contracting, the pressure is highest. This pressure is called the systolic pressure.
When, the heart relaxes between each contraction, the pressure declines. When
the heart is at its utmost calm state, the pressure is lowest. And this is diastolic
pressure. The next stages will help you measure blood pressure precisely.

1. Wash your hands and prepare the paraphernalia you will need.
2. Acquaint yourself and let the patient/client tell the method to be done.
4. Request your client to rest silently. Have him/her lie down or sit on a
chair any comfortable for him/her.

6. Expose the arm of your client by rolling the sheaths up. Take your
client’s arm from the elbow down to rest fully stretched on the bed or the arm
of a chair.
7. Unfold the cuff, release the screw and squeeze the cuff with your
hands to take away air entirely.
8. Cape the cuff around your client’s arm above the elbow, not too tight
or too slack.
9. Look your client’s brachial pulse at the inside of the elbow. Hold the
diaphragm there and bloat the cuff until the pulse fades. Take note of the
reading and immediately decrease the cuff. This is the client’s approximate
systolic reading and is called the palpated systolic pressure.
10. Put the stethoscope’s earpieces into your ears and put the diaphragm
on the brachial pulse.
11. Turn the screw to close it. Bloat the cuff until the dial points to 30 mm
above the palpated systolic pressure.
12. Turn the screw to open it. Let the air release slowly until the sound of
the pulse comes back. Take note of the calibration that the pointer passes
as you receive the first sound. This specifies the systolic pressure.
13. You have to continue freeing the air from the cuff. When you receive
the sounds change to somewhat softer and faster and vanish, take note of
the calibration. This is now the diastolic pressure.
14. Deflate the cuff totally. Take it from the arm of your client and take
the reading on the client’s chart.
15. Rub the earpieces of the stethoscope with an antiseptic pad and put
the equipment back to their proper place and wash your hands.
