1. The undersigned would like to formally express her intent to be a Cadet

Officer Candidate and continue her duties for the Corps Year 2014 – 2015.

2. The undersigned would like to pursue to become a Cadet Officer because

she believes that she can still contribute to the growth and development of the UP Corps
of Cadets by continuing to fulfill her duties.

3. The undersigned assures that best efforts would be given to any duties
assigned to her as a Cadet Officer Candidate.
