AFFIDAVIT OF TWO DISINTERESTED PERSON FOR LATE REGISTRATION

We, ELMA K. GARAS and LEONA D. SINGA-NGAN, both of legal age, both
married, Filipinos, and both residents of Camp 4, Tuba, Benguet, Philippines, after being duly
sworn to law, hereby depose and say:

1. That we know personally know CRVYN BLAIR WANDAS BAD-AY who was
BORN ON MAY 21, 2012 at TUBA, BENGUET and his parents are GREGORIO
MAGADIA BAD-AY and ESTELA BOKO WANDAS;

2. That when his mother went to NSO to secure a copy of his Certificate of Live Birth,
she was issued a NEGATIVE CERTIFICATION OF BIRTH that her son was not
registered or the facts of his birth was not duly registered;

3. That the reason his facts of birth were not duly registered with the Office of the Local
Civil Registrar is that both of his parents are busy working during that time and did
not bother to register it;

4. That we know the fact of his (child) birth because we were one who witnessed it and
the one who help the family in their needs at the time of his birth;

5. That we know the above facts and circumstances because we are long time family
friends and neighbors of the above named person and his parents;

6. That we are not in any way related to the above-mentioned person neither by
consanguinity nor affinity;

7. That we are executing this affidavit to attest to the truth of the foregoing facts and to
support the application of his parents for his late registration of the facts of his birth
and for all other legal intents and purposes.

IN WITNESS WHEREOF, we have hereunto affixed our signatures this 11th day of
September, 2014 at Baguio City, Philippines.
