- Manpower for ballast & de-ballast were not enough to cover the work activities. ML, GP
ML suggested that company should hire additional manpower through agency.

1x6 Axles), Thailand ± PK1 26 Axles (2x4 & 3x6 Axles), SDE ± PK4 32 Axles (2x4 & 4x6

! PTMI Batam ± 4 Pumps with 8 hoses, SDE ± 14 Pumps with 28 hoses. N/a

- Hoses for ballast activity were not enough as some of the hoses were damaged EH, ML,SH
- RC suggested that company buys cheaper hoses use for discharge.

 Roles & Responsibilities, Competence & Training

Suggest conducting in-house training for the NIOSH-OSFAM Safety Passport. To Check NM April
with the training provider whether they are available or not.

Minimum of 4 persons to conduct Basic Rigging and Slinging Training by MSTS Asia. NM NM April
to arrange operation crews to attend the course.

 Customer Feedback & Satisfaction

PTMI Project Team praised ALE Malaysia for the excellent job performed on the Info
Peregrino Project.

ALE Malaysia to request hardcopy Appreciation Certificate for records.

 Integrated (HSQE) Management System & Documentation

Re-branded IMS documentation now available on website. Info

This will be part of compliance audit on how to access to the IMS on the website All

Email regarding the new IMS had been sent to all staff for their use and reference. NM

Audit performed by John Fitzmaurice at Ramunia and Thailand was not NM May
closed yet.

HSQE Advisor should come over to Ramunia and Thailand for corrective actions. NM April

Committee suggested that HSQE Advisor should come over to Vietnam for audit and NM April
HSQE meeting.

Internal audit will be conducted every month by HSQE Advisor. Info

 Status of incident investigations, preventive and corrective actions

Currently no incident and accident occurs for the month of February. Each region for SEA Info
(Thailand and Vietnam) has to report to HSQE Advisor if there is any incident or accident
for investigations and corrective actions.

 Evaluation of compliance to applicable legal and other requirements

Diesel Tank Storage ± Application was submitted by HSQE Advisor to PG BOMBA ABT NM Closed
Azizul (from licensing section). 14 days window allowance required by BOMBA prior to
inspect the storage area.
- BOMBA PG had inspected the tank and License Support Letter had been
obtained from them.
- NM to apply with KPDNHEP (Kementerian Perdagangan Dalam Negeri & Hal NM On going
Ehwal Pengguna) for the Storage Permit.

HSQE Advisor has to attend ISO and OHSAS training due to maintain documentations NM May
and to perform internal audit in the regional office.

Suggested to install steel barrier at security post guard. Proposal and design will be ML, HA, May
submit for approval EH
