You have to admit you made a mistake to your team.

At this point in the scenario, I chose to be a welders assistant in a


company that specializes in

making gas and oil separators for the oil and gas industry. My job
responsibility is to assist the

welders in my department and insure that the pipes and flanges that
help transfer the oil and

gas, and torqued to Specification and aligned properly and precisely. In


as well to in sure check

and ball valves are in the correct position and level for fluid transfer. I
have been a welder's

assistant now for 3 months and have acquired the skills to effectively
do my job and maintain

safety. Recently however, I have been trying to progress the amount of


separators we complete

in a week's time that resulted in not torquing down three of the


separators check valves and

when tested for pressure they failed and one check valve exploded
when under pressure.

Upon quality control investigation they found that these check valves
:
were Under torqued and

miss aligned, this was my responsibility and alone my mistake. I now


have to address my fellow

employees about said mistake. I would call a meeting and have us all
meet in Our designated

meeting room. The meeting would go as Followed. “As you all know
three of the separators

failed during a pressure test at the check valves and ball valves and one
resulted in the

explosion of a check valve. As quality control did investigation under


the cause, They found it to

be that ball and check valves we’re under torqued as well as


misaligned. Considering the high

pressure volume that these check valves have to go through in order to


pass by the bolts not

being torqued down on the flanges to the precise specifications it was


not a tight enough seal

and that was the cause of the accident that occurred yesterday. I
wanted to bring everybody into

this room and apologize for my carelessness and miss handling of the
important Products and

endangering the health or lives of others. I will say that these actions
:
were not intentional. I've

simply just relaxed my expectations upon my job performance. And I


apologize for my actions

and I will do more to be professional and coherent when working


around others and with my

duties in this field”

This is simply An example of what could happen in the oil and gas
industry regarding the

production of oil and gas separators. This is the scenario that I chose,
and given the mistake

how I would’ve addressed My fellow coworkers and team.
