MEMORANDUM OF AGREEMENT

This MEMORANDUM OF AGREEMENT is made and entered into by and among:

WHEREAS, the DOST CALABARZON promotes adoption and utilization of

science, technology and innovation developed or publicly-funded by government and
coordinates the delivery of appropriate technologies to the communities in the region;

WHEREAS, the DOST CALABARZON implements the STARBOOKS, one

of the programs included in the National Priority Program List of DOST, a digital library
that contains thousands of digitized science and technology resources in various formats
(text and video/audio) placed in specially designed “pods” set in a user-friendly
interface; aims to create interest in the field of Science and Technology which may
increase the number of Filipinos enrolling in S&T courses;

WHEREAS, INITIALS OF SCHOOL(ex DOST)aims to provide excellent,

equitable and relevant educational opportunities in sciences and technology through
quality instruction and responsive research and development activities; commit to extend
their full cooperation and support for the effective and efficient implementation of the
aforesaid project;

WHEREAS, INITIALS OF SCHOOL(ex DOST), has expressed interest to

adopt STARBOOKS Digital Library to enhance their current library services by
increasing access to science and technology information;

WHEREAS, the parties to this Agreement, have agreed to work together to

accomplish the objective of deploying STARBOOKS to its beneficiaries and upgrade its
school library by adopting the STARBOOKS Digital Library developed by DOST-
Science and Technology Information Institute;.

NOW THEREFORE, for and in consideration of the foregoing premises and of

the mutual covenants and stipulations hereinafter set forth, the parties hereto agree to
enter into this Memorandum of Agreement under the following:

TERMS AND CONDITION

1. That DOST CALABARZON thru PSTC-Cavite shall:

1.1 Provide technical assistance for the implementation of the project entitled

software and cover the expenses that will be incurred in the conduct of training
orientation ;

1.2 Monitor the progress of utilization of STARBOOKS and timely

submission of transmittal report generated by the system;

2. That INITIALS OF SCHOOL(ex DOST)shall:

2.1 Provide the ____ computer unit(s) to be installed with STARBOOKS

software and will serve as the server;

2.2 Designate two (2) point persons (preferably librarian and IT staff) that
will monitor, manage, maintain and operate the digital library to ensure
sustainability of the STARBOOKS project;

2.3 Designate a team that will monitor, manage, maintain and operate the
digital library;

2.4 Monitor the use of STARBOOKS and submit TRANSMITTAL report

generated by the system via Online Account that will be provided during
the training. Failure to submit for three (3) consecutive months will cease
to receive content updates and upgraded version;

2.5 Estabish the guidelines on the use of STARBOOKS to ensure its

efficient and maximum usage;

2.6 Allow free access of STARBOOKS to teachers, students and interested

groups/individuals in accordance to established guidelines;

2.7 Provide supplies and materials that may be needed such as, but not
limited to, printer, paper and ink supplies to complement the operation of
the STARBOOKS system;

2.8 Acknowledge DOST CALABARZON in any media release,

publication, progress and other reports pertaining to the operation of
STARBOOKS;

2.9 Provide access to DOST CALABARZON during the project visit and
monitoring; and

2.10 Be responsible and accountable for the repair, maintenance and

safekeeping of equipment and properties ;

3. OWNERSHIP OF EQUIPMENT AND NON-CONSUMABLE

Restrictions on Use and Transfer of STARBOOKS or the DOST Science Digital
Library Kiosk. The DOST STII is the owner of the design and contents of the
DOST Digital Science Library Kiosk.

5. AMENDMENTS
This Agreement or any part or portion hereof shall be amended, modified or
revised only through formal written agreement between the Parties hereto citing the
specific clause(s) and/or provisions to be revised and their stating the corresponding
amendments.

6. EFFECTIVITY AND DURATION

This Agreement shall take effect upon signing by the parties hereto and shall
remain in force for one (1) year commencing upon the receipt of assistance,
renewable for another year thereafter, unless sooner terminated by either party
provided that a written notice shall have been served one month prior to its
termination.
