Optistar Elite Alarm 15

Reason for this bulletin

Alarm 15, (Changed Syringe Size Alarm), occurs intermittently when loading a
syringe, or during an injection.

Description

An issue with the plastic molding of the syringe holder nest has been identified on
injectors manufactured prior to September, 2012 which may cause an intermittent
Alarm 15 to occur due to improper seating of the syringe size sensing PCBs.

A change was implemented on the nest on 8-27-2012 and subsequent new


manufacturing tooling has been made to eliminate the occurrence of this issue,
however, injectors made prior to this date still may have the possibility to exhibit the
above stated issue.

Correction

If an injector is getting an intermittent Alarm 15 and the injector was manufactured


prior to September, 2012:

 First replace the Syringe size sensing PCBs, p/n 802313-1.


 If the above does not rectify the alarm, replace the entire nest with the Syringe
holder service kit, p/n 802748.
