NOTE: CALL BCO BEFORE FILLING UP, LET HIM KNOW YOU GOING TO FILL UP. GET LOG BOOK OUT OF COMPANY MAN OFFICE AND KEEP UP WITH GALLONS PUMP ON METER.

DESCRIPTION OR HAZARD THE HAZARD

(1). Pre-Job planning & safety meeting (A). Bad communication, miss guidance (A). Pre-Job Safety Meeting and JSA

(2). Stretch hose to equipment to be (A). Worn out hose, (A). Check hose on a regular basic
fuel up (B). Leak in connection (B). Check connection before fueling
(C). Kinked Hose (C). Make sure hose is free of kinks
(D). Hose rupturing, (D). Keep clear of engine exhaust
(E). Trip Hazards (E). Do not run hose in walkways

(3). Begin pumping (A). Tank overflows, Potential spills, pollution (A). Stay alert
(B). Diesel spraying in face, skin, clothes (B). Open valve slow, wear proper ppe, If you get diesel on clothes, change
clothes

(4). Clean up & return hose to rack (A). Spills (A). Turn valves off, drain hose in fuel tank. Install plug on hose
(B). Nozzle or hose could get caught creating frays in hose Clean up any spills w/ absorbent pads, and dispose of in proper place
or weaken the integrity of the hose. (B). Wrap hose neatly on rack, keep free from sharp edges to prevent getting cut.
Return nozzle to drip pan.
