AFFIDAVIT OF UNDERTAKING AND PROMISSORY NOTE
I, NICHOLEE JHAINE B. LABADAN, of legal age, Filipino, married and a
resident of Barangay I, Vigan City, Ilocos Sur after having been duly sworn to in
accordance with law, depose and state THAT:
1. I am presently employed as Administrative Aide IV of the City Government
of Vigan-City Engineering Office;
2. As a government employee, I am entitled to my mid-year bonus provided
that I possess no disqualification in receiving the same;
3. The mid-year bonus was released on May 15, 2017;
4. The amount of TEN THOUSAND SIX HUNDRED EIGHT PESOS
(₱10,608.00) was then credited to my account representing my mid-year
bonus;
5. I withdrew the said amount immediately with the belief that I am entitled
thereto;
6. I was belatedly informed that the HRMO committed a mistake in including
me in the payroll for mid-year bonus, hence the promise to return the same
to the City Government of Vigan through City Treasurer’s Office;
7. I promise to pay a minimum of FIVE HUNDRED PESOS (₱500.00) every
pay day starting June 15, 2017 until such time that I will settle the full
amount of TEN THOUSAND SIX HUNDRED EIGHT PESOS ( ₱10,608.00)
as payment for receiving my mid-year bonus for 2017;
8. I am executing this affidavit to attest to the veracity of my statements and
for all legal intents and purposes.

4. That such, I intend to loan from COVE the amount of Fifteen Thousand
Pesos (₱15,000.00) in order to pay the above-mentioned loan from Rang-
ay Bank;
5. I promise to pay the amount of One Thousand Five Hundred Pesos
(₱1,500.00) monthly until fully paid in favor of the Organization;

Services Office;
2. As a government employee, I am a member of the City Organization of
Vigan Employees (COVE);
3. That I incurred a loan with outstanding balance of Six Thousand Pesos
(₱6,000.00) to the above-mentioned Organization;
4. At present, I am paying the said loan in the amount of One Thousand
Pesos (₱1,000.00) monthly;
5. However, due to uncontrollable circumstances, I cannot fulfill to pay the
said amount as promise but instead, I promise to pay the amount of Nine
Hundred Pesos (₱900.00) monthly until fully paid in favor of the
Organization;
6. I am executing this affidavit to attest to the veracity of my statements and
for all legal intents and purposes.
