AFFIDAVIT OF INSURANCE CLAIMS

I, Leandro Baldemor, Filipino, of legal age, married a resident of barangay Guadalupe,

Cebu City, Philippines, after having duly sworn to in accordance with law, do hereby
depose and say that:

1. I am the owner of the motor vehicle, a BMW K 1600 B with registered plate
number LBF 1715

2. I was cruising on my bike on the 12 th of March 2021 at Fuente Osmena Rotonda

when a Ceres Bus hit me and caused damage

3. After the incident I called the Cebu City Traffic Management Office to investigate
and determine which party is at fault.

4. The Investigative report by the Cebu City Traffic Management Office postulates
that Ceres Bus was at fault.

5. I proceeded to the BMW Cebu- Autowelt located at Nivel Hills, Cebu Veterans
Drive Cebu City, to have the Motorcycle checked, assessed and quote the
damages.

6. The Diagnostic report shows the amount of 550,000 in Philippine currency to

have the BMW K 1600 B repaired. Report signed by BMW Manager.

7. I am executing this affidavit for the purpose to establish the truth and veracity of
the foregoing facts in order to avail and claim of the Insurance benefits in order to
be assisted in the repairs and of the BMW motorcycle Bike

AFFIDAVIT OF DESISTANCE

I, Leandro Baldemor, Filipino, of legal age, married a resident of barangay Guadalupe,

Cebu City, Philippines, after having duly sworn to in accordance with law, do hereby
depose and say that:

1. I am the owner of the motor vehicle, a BMW K 1600 B with registered plate
number LBF 1715

2. That the 12th of March 2021 at Fuente Osmena Rotonda a Ceres Bus hit my
bike and caused damage

3. After the incident I asked the Cebu City Traffic Management Office to investigate
and determine which party is at fault.

4. I proceeded then to the BMW Cebu- Autowelt located at Nivel Hills, Cebu to have
the Motorcycle checked, assessed and quote the damages, which cost 550,000
in Philippine Currency

5. After the assessment made by BMW- Autowelt, I called my lawyer and narrated
to him the incident, and told him to file a civil case for damages against Ceres
Bus

6. I then contacted to my Insurer to inform them, and they told me that they will
cover the amount of 350,000 in Philippine Currency

7. On the evening of March 12 an unexpected call from Ceres Bus reached me and
informed me that they were willing to pay the balance not covered by the
Insurance company, but only if I would desist from filing the suit for reckless
imprudence

8. Thus I am executing this affidavit for the purpose of establishing the veracity of
the foregoing facts and in accordance with our agreement with Ceres Bus to
desist from filing the Civil Suit for Damages so that the Latter will be shouldering
the balance not covered by the Insurance Company.

AFFIDAVIT OF QUITCLAIM

I, Leandro Baldemor, Filipino, of legal age, married a resident of barangay Guadalupe,

Cebu City, Philippines, after having duly sworn to in accordance with law, do hereby
depose and say that:

1. I Leandro Baldemor, employee of Big Boy Construction for 15 years will avail of
the early retirement program offered to me by the Company

2. I have freely decided to resign from the Big Boy Constructions effective March 15
2021.

3. I, hereby acknowledge upon availing of the program and with my awareness and
consent will receive the amount of 2,500,000 in Philippine Currency and is
satisfied and contented by it.

4. Furthermore, I manifest and voluntarily declare that I have no cause of action or

claim whatsoever against Big Boy Construction its officers, agents, or assignees.
As a consequence thereof, I shall desist from filing any complaint, civil, criminal
or administrative, against Big Boy Construction its officers, agents, or assignees
in, including but not limited to, all government/administrative agencies,
departments, offices, quasi-judicial agencies, the courts and tribunals of the
Republic of the Philippines.

5. I declare finally that I have read the contents hereof fully and I completely
understand the same and/or that the contents hereof have been fully read and
explained in a language or dialect known to me before signing the same and this
Affidavit of Quitclaim/Release is made voluntarily and willingly with full knowledge
of my rights and privileges under the law;
