Note:

1. Only examinees are allowed to enter the testing venue.

2. Be at the vicinity of the testing venue at least 30 minutes before the start of the exam.
3. Upon entering the gate of the testing venue, all examinees will undergo COVID-19 symptoms screening. Those who exhibit
symptoms will not be allowed to proceed.
4. Report to the testing venue and present this permit with your ID or Certification from the Barangay Chairman stating that
you are currently enrolled as Grade 12. No ID/Certification, No exam..
5. Wear facemask and face shield at all times. Entry will be denied for the non-compliant.
6. Bring your own pencil (#2) and snacks. You will not be allowed to go out of the testing venue once the test has started.
7. Calculators and cellphones are not allowed. These must be safely stowed within your bags or handed over to the room
examiner or proctor for safekeeping.
8. Examinees who need to use the comfort room / toilet facilities while the exam is on-going must ask the permission of the
examiner or proctor. The examinee will be accompanied by either of them up to the entrance of the facility.
