Further to your recent interview, I have pleasure in confirming our conditional offer of appointment to
the above.

A summary of the main terms and conditions of employment are attached to this letter.

This offer remains subject to satisfactory receipt of the following pre-employment checks. If you do
not provide the information requested below, then the offer may either be withdrawn or your
employment terminated immediately, as appropriate:

• Confirmation of Employment/Education/Unemployment (previously referred to as


references), which the Trust considers satisfactory, covering the last 3 years. If your most
recent employer is an NHS organisation, we will seek the confirmation from them only. The
confirmation of employment will be sought from your current and/or previous employers'
HR departments. Please ensure we have their contact details including telephone number
and work related email address. Please advise your referees that we will be contacting
them to ensure swift recruitment process.
• Verification of your Professional Registration Status (if applicable)
• Occupational Health Declaration - All applicants for posts at the Trust are required to
make a health declaration as part of the recruitment process. Please go to

begin the process as soon as possible.

If there is any part of the new role that is likely to have an impact on your health you will be
contacted by a member of our Occupational Health Team to discuss this. Those staff who
are going to be working in direct contact with patients, or undertaking Exposure Prone
Procedures, will be contacted for additional information.

Employee Awareness Statement - Right to work in UK

If your right to work in the UK is limited as the result of your visa, we are obliged to undertake regular
checks in the form of both an annual right to work check, on the anniversary of your appointment, and
a review when your current right to work in the UK is due to expire.

In addition, you are required to keep the Trust updated as to any changes in your personal
circumstances, address, marital status, study plans, working hours, second jobs etc. as these may all
impact on your right to work in the UK.

We will endeavor to give you fair warning of the upcoming anniversaries and the expiry of your current
right to work. However, you are responsible for ensuring that your right to work in the UK is
maintained and for making any applications, in good time, when your current right to work expires.
You are required to comply with our requests to provide evidence of your ongoing right to work.
Failure to comply with these requests may result in suspension with or without pay, or potentially
dismissal.

In order for us to get you set up promptly please complete, save as an unprotected Excel Workbook
or Excel 97-2003 document and return as soon as possible the attached forms in the excel workbook
in full and email them to rf.newapplicants@nhs.net. Please ensure to include your Applicant ID in the
subject header when returning your forms.

HMRC about you and help them use the correct tax code.

- Model Declaration Form A (for posts requiring a DBS) or Model Declaration Form B (for posts not
requiring a DBS).

Your employment may be subject to a probationary period of six months (or less, depending of the
length of the contract you will hold with the royal Free Hospital). Please note that during this time,
your employment may be terminated with one week’s notice on either side. Thereafter, notice to
terminate your employment is as set out in the Summary of Offer statement below.

Please confirm your acceptance of this conditional offer by simply replying to the email sent to
you with the attached conditional offer. Please retain a copy for your own records.

I will keep you informed of the progress of your application and ask that you do not hesitate to contact
me if I can be of any further assistance at this stage. Once all pre-employment checks have been
completed to our satisfaction, I will write to you again to confirm this and I will then send you copies of
your Statement of Main Terms and Conditions of Employment, and details of the Corporate
Orientation Programme that you will be required to attend. You will need to agree your start date with
your new line manager.

Pension: You will automatically be enrolled into the NHS Pension Scheme. Contributions are
deducted at source at the rate of between 5% and 14.5% of salary depending on your
