and sealed by duly licensed professionals and signed by owners

One-Stop Shop for Construction Permits (OSCP)/ 2nd
Four (4) copies of filled-out Application Form for Electrical Permit
Floor, GSC Investment Action Center, City Hall Compound

Three (3) Photocopies of valid PRC ID & current PTR with seal and
3 specimen signatures of the professional by the professionals and Assigned Professionals
signed by the applicant/ owner/s)
p Two (2) sets of Cost Estimate/ Bill of Materials (signed and sealed
by duly licensed professionals and signed by owner/s) Assigned Professionals
