1) Industrial Training Log Book
❖ Students have to print this log book and binding it using comb binding. It is encourage to put plastic
cover at the front and back page of the log book.
❖ Complete the detail required on the first page of log book.
❖ It is student’s responsibility to make the main entries of the log book and keep it up to date.
❖ Students must ensure that:
i. The log book is available at workplace during the training.
ii. All entries should be written understandable and in English.
iii. All entries, except sketches, are made in ink.
iv. Entries are made within a week of the work to which they refer.
v. Entries must be regularly initialled by your Industrial Supervisor.
vi. The log book is handed to your Industrial Supervisor for retention on your return to UiTM and this
will later be handed to the Academic Supervisor and Internship Coordinator for grading.
3) Report of Duty
❖ Students must report duty based on date and time as decided by company and Internship Coordinator.
❖ Please wear a neat and an appropriate work attire to the workplace.
❖ Please bring along the following documents during report of duty:
i. Passport Picture
ii. Your Resume
iii. BLI03 or Offer Letter.
iv. SLI03 (Surat Penerimaan Menjalani Latihan Industri).
v. BLI04 (Industrial Training Report Duty Form (Borang Lapor Diri Latihan Industri) )

4) Important Rules About the Industrial Training
❖ All students should make considerable effort and give sufficient thought into obtaining the most relevant
and effective Industrial Training in order to achieve the objective of the industrial Training.
❖ The selection for place/company for Industrial Training shall meet the following criteria;
o The organisation and the training programme/project that offers are acceptable by the Faculty.
UiTM.
o The organisation is able to provide supervision and conduct assessment on the student during
the training period according to the Faculty evaluation system.
o The duration of the industrial training meets the course requirement.
o The student is not associated with the company and has no immediate family members, close
relatives or friends either working in the organisation or issuer director's) or substantial
shareholder's) of the organisation which may give rise to conflicts of interest. Failure to abide
these requirements will render the industrial training invalid.
❖ Student attending industrial training is considered as a full-time UiTM student thus must abide by the
University rules and regulations. Beside, the student must obey the regulation that has been enforced
by the respective company.
❖ Student must give full attention and behave professionally as a university student during the training
period.
❖ Student must attend the training on time according to usual office hours of the company. In case of
emergency, the student must inform the supervisor and the coordinator of industrial training as soon as
possible with a valid document.
❖ Student is not allowed to misuse company's properties.
❖ Student who receives allowance from the company must inform the coordinator industrial training.
❖ Student who behaves unconditionally and against the company rules and regulation can be summoned
to attend a trial in the university court for further action.
❖ Student is expected to keep the secrecy of the company while and after attending the industrial training.
❖ Student is not allowed to take any leave during industrial training period. Occasionally, in case of
emergency or sickness, a valid Medical Certificate (MC) or a valid leave’s approval from the industrial
supervisor must be submitted to the supervisor and the Faculty.

Failure in Industrial Training
❖ Students may fail their industrial training under any of the following condition;
a) poor evaluation and comments from industrial supervisor and visiting lecturer.
b) do not follow Industrial Training procedure.
c) late or fail to submit final report to the Academic Supervisor and Internship Coordinator.
d) absence to industrial training without permission from the industry.
e) changed industrial training placement without permission from the faculty.
f) do not comply to industrial rule and regulations.
g) any act or behavior that against the UiTM regulation.
5) Academic Supervisor
❖ A member of Faculty of Computer Science and Mathematics, UiTM.
❖ Each academic supervisor will be assigned 2 to 5 internship students in each semester.
❖ Students have to inform and introduce themselves to the academic supervisor through email and
Whatsapp by providing the following details:
1) Fullname and UiTM ID,
2) Hp Number and email address,
3) Duration of Internship,
4) Company and Address of Internship,
5) Industrial Supervisor’s Name and Contact No.
!!!! Students must provide the student’s details when emailing the academic supervisor especially in
submitting industrial training logbook, report and etc.
❖ Academic supervisor will check the email, industrial training logbook and report starting from Week 6
until you have completed the industrial training and contributes 70% of industrial training assessment.
6) Industrial Supervisor
❖ Industrial supervisor has to complete BLI 04 form in the first week of internship.
❖ Industrial supervisor will check and verify industrial training logbook regularly.
❖ Student must adhere to the industrial supervisor instruction regarding to any work related assignment
during the internship.
❖ Students must maintain good rapport with the industrial supervisor /company.
❖ Industrial supervisor/company will fill in declaration form, BLI 05 and ITAR form – link will be
provided by internship coordinatoor.

7) Assessment of Industrial Training
This Industrial Training assessment will be based on evaluation of the logbooks and final reports, evaluations
by industrial supervisor and academic supervisor. Contributions of each assessment are as follows;

Students are required to submit a softcopy of BLI 04 to the Internship Coordinator
within a week after registration at the company. Student who failed to submit BLI
04 will be considered as not undertaking the industrial training program.

BLI05 – Academic Supervisor Evaluation (70%)
Students should ensure that the log book contain the following information:
i. A neat and concise description of each your training locations and the
work on which you are engaged.
ii. Include duties you have performed, facts and procedures you have
learned, skills you have mastered and observations you have made.
iii. Provide relevant evidence of sketches, data, memo and etc
iv. References to textbooks, standards and other technical information
related to the work being under taken.

Statement of Absence (if any)
Students are compulsory to fill the form if they are absent during the Industrial Training period and get approval
from the Industrial Supervisors.
