the ‘ETHOS HEALTHCARE PVT LTD”) is looking forward to have a business relations with CMPak Limited
(the “Zong”) on long term basis and thus require 05 connections for GSM /MBB Devices / ESS solutions
(please specify whichever applicable). I Mr. / Ms. Muhammad Hassan Iqbal Raad bearing
CNIC/Passport # 4230188166285 working as Account Manager being an authorized signatory do hereby
accepts the following terms and conditions stated therein the proposal and assures that ETHOS
HEALTHCARE PVT LTD will;
 Be responsible to issue these connection / hardware to its employees.
 Be the direct user of these services and takes complete usage responsibility.
 Be responsible for the payment & clearance of dues against each number without failure on or before of
the due date as per the billing.
 Keep Zong informed of any future change in the authorized signatory/POC, Customer’s and / or user’s
address for the purposes of billing and update of record / credentials.

Customer requests that security deposit with regard to the Post Paid connections may kindly be waived off and
credit limit may be fixed / locked as mentioned in below table;

This LOI shall be construed and governed in accordance with the laws of the Pakistan and the obligations, rights
and remedies of the parties hereunder shall be determined in accordance with such laws.
