1.0 BACKGROUND
Main Operation : BOP Test.

Nature of Incident : Could not retrieve BOP Test Plug after BOP test failed.

Location : Iraq - Basra - Zubair oil field ZB-347

Date of Incident : 10th February 2018

2.0 SEQUENCE OF INCIDENT


 Post installation of section A “CHH”, TechnipFMC Engineer ran in Bop test plug
for Bop Test.

 During Testing driller observed a pressure drop.

 TechnipFMC Engineer tried to pull out TP to inspect and change S-seals.

 Gradual over pull up to 15 ton was given but unsuccessful.

 N/D BOP for visual check and found Test plug move vertically up to 10 cms and
stop.

 Tried to rotate and over pull to release.

 Once Test plug released TechnipFMC engineer found CHH bowl area was
scratched.

 Seals of BOP TP found damaged too.

3.0 FIELD FIX / CORRECTIVE ACTION


 Customer resumed further operation as is.

4.0 CONTRIBUTING FACTORS


 TDS misalignment.

 Probable cause of degraded seals due to improper storage.

 TDS was used to rotate the stuck plug

 Inadequate communication of the issue to the base while it happened.

5.0 ROOT CAUSE


 Misalignment overlooked considering the operation to be routine.

 Even though the seals were checked and appeared good; possible degradation of
the material due to extended exposure to the atmospheric elements.

6.0 PREVENTIVE ACTION


 Confirm the alignment prior to running of TechnipFMC equipment.

 In field Issues to be communicated back to the base immediately.

 Seals to be checked effectively for any traces of damage or replace when in doubt.

 Storage of elastomeric to be carried out as per procedure and the procedures to


be shared with the customers.

 Use elevators to run BOP Test plugs.

 All service personnel notified of the issue and requested to be extra cautious while
using of customer stored equipment.
