VI. RIGGING AND HOISTING EQUIPMENT

Are shackles, eye bolts, hooks and blocks in good conditions?
Are the Chain blocks / Lever hoist (come-a-long) in good conditions?
Are the slings used in worn, broken and damaged conditions?
VII. LADDERS AND SCAFFOLDINGS
Are the appropriate scaffolds materials used and in good conditions?
Are good housekeeping maintained above walkway and platform?
Is the scaffold operation is adjacent to overhead power lines?
Is base of scaffold 1.5 times the depth away from edge of excavations?
Are the tools used for scaffold erection and dismantling are as per the
manufacturers recommendations?
Are the wheel casters of mobile scaffold locked while scaffold in use?
Are the scaffolds erected is inspected and tagged?
VIII. WORK PERMITS
Is joint inspection conducted while Work Permit is accepted?
Does the acceptor of the Work Permit review the scope of job with the
craftsman?
Are requirements on permit being followed?
IX. HAND AND POWER TOOLS
Are the tools maintained and stored properly?
Are the tools equipped with mandatory safety guards?
Are power tools (lathe, cutting machine, pipe toreadors, compressor,
etc.) clean and in good condition?
X. ARC AND GAS WELDING
Are high-pressure cylinders being stored properly, or placed in cylinder
racks and carts?
Are regulators being removed from cylinders and stored properly when
not in use?
Are welding cables being stored properly?
Are oxy-acetylene hoses in good condition?
Are "flashback arrestors" being used on the oxy-acetylene system?
Are welding curtains and shields being used during welding and
grinding?
