BASIC JOB STEPS POTENTIAL HAZARDS RECOMMENDATIONS


Preliminary steps for the job  Permit issuer & Permit receiver should conduct
effective joint site inspection before & after job.
 Permit shall be obtained prior to commence
 No valid permit to work the task.
 JSA shall be available on site prior to start the
work.
 Make sure the gas test results are satisfactory
and safe for the use of scissor lift & boom lift.
 Task shall be clearly communicated among

Use of step ladder  Uninspected, poor, damage step ladder  Ensure that ladder is inspected, good condition

 Ensure the stiles are not bent or damaged.
 Ensure that rungs should not be bent, missing,
loose.
 Ensure the locking bars are engaged.
 Do not use aluminum step ladder for inside
sub-stations or high voltages rooms & for live
electrical work.
 Persons may fall from ladder  Ladder must be placed on firm level ground,
and ensure that ladder legs are fully extended.
 Maintain three point contact system.
 One person is allowed in one time.
 Do not use top two rungs.
 Always grip the ladder and face the ladder
rungs while climbing or descending—don’t slide
down the stiles.
 Avoid holding tools or other object while
climbing.
 Avoid over-reaching.
 Ladder feet should be fitted with rubber for
strong grip with ground.
 One person should hold the ladder while
working on ladder.
 Do not lean ladder with wall or any other
object.
 Do not place ladder on any object.
 Persons must be trained in order to work on
step ladder.
Use of scaffolding for work at height  Uninspected, poor erected, missing  Scaffold must be inspected by authorized

obstruction, do not keep unnecessary
tools/material on platform & all tools/material
must be well arranged in a side.
 Improper method of getting material  Use canvas basket & nylon rope for transfer
tools & light material.
 Use pulley system.
 Do not throw any tools/material from ground
to height and from height to ground.
 Personnel injury  Use all mandatory PPEs all the time along with
full body harness.
 Peoples must use chip strap while work at
height.
 Scaffold may collapse  Provide barricade with warning signs around
scaffold.
 Before every use, ensure that scaffold is in
stable condition & all components are in place.
 Scaffold must be re-inspect after hit by any
moving equipment, alteration, or heavy winds.
 Unauthorized modification or removal of  Erection, dismantling & modification is only
scaffold component allowed to certified scaffolders under the
supervision of certified supervisor.
 Unauthorized persons are strictly not allowed
to modify any component of scaffold.
Use of scissor lift for work at height  Unauthorized use  Scissor lift must be operated by certified
operator; he must hold valid TUV for the
specific equipment.
 Remove Ignition key (if fitted).Lock all
cabinets/covers when leaving the machine.
 Poor working environment  Scissor lift must be use in well ventilated area.
 Do not operate scissor lift in bad weather e.g.
heavy winds, heavy Rain, thunder and lightning
etc.
 Must be operated on firm level ground.
 Provide barricade with warning signs around
working area.
 Fire & explosion  Gas test results must be satisfactory.
 Remove all combustible/flammable material
from area, or cover with fire blanket.
 Never refuel the machine whilst it is being
operated or the engine is running.
 Always refuel the machine in a well ventilated
area.
 Ensure there are NO naked flames or ignition
sources.
 NEVER smoke during refueling.
 Avoid spilling fuel and immediately clean up
any spilled fuel.
 Collision with pedestrians, other vehicles  A trained flag man must be used while
