BASIC JOB STEPS POTENTIAL HAZARDS RECOMMENDATIONS

Preliminary steps for the job
 No valid permit to work
 Miscommunication
 Permit issuer & Permit receiver should conduct
effective joint site inspection before & after job.
 Permit shall be obtained prior to commence
the task.
 JSA shall be available on site prior to start the
work.
 Make sure the gas test results are satisfactory
and safe for the use of scissor lift & boom lift.
 Task shall be clearly communicated among
whole site team prior to start any activity.

 Tool box shall be carried out prior to start job.

 Conduct Safety Task Assessment to discuss job
 Personal Injury due to lack of safety and control measures.

correctly.
 Wear proper PPE prior to start the job.
 Ensure that ladder is inspected, good condition
& all components are in place.
 Ensure the stiles are not bent or damaged.
 Ensure that rungs should not be bent, missing,
loose.
 Ensure the locking bars are engaged.
 Do not use aluminum step ladder for inside
sub-stations or high voltages rooms & for live
electrical work.
 Ladder must be placed on firm level ground,
and ensure that ladder legs are fully extended.
 Maintain three point contact system.
 One person is allowed in one time.
 Do not use top two rungs.
 Always grip the ladder and face the ladder
rungs while climbing or descending—don’t slide
down the stiles.
 Avoid holding tools or other object while
climbing.
 Avoid over-reaching.
 Ladder feet should be fitted with rubber for
strong grip with ground.
 One person should hold the ladder while
working on ladder.
 Do not lean ladder with wall or any other
object.
 Do not place ladder on any object.
 Persons must be trained in order to work on
step ladder.
 Scaffold must be inspected by authorized

 Always check inspection tag before use of

conditioned full body harness.
 Full body harness’s snap hooks must be tied
with anchor point all the times.
 Do not climb on toe board & guard rails.
 Stay only within platform. Do not climb on
structure or any other object.
 Ensuring that information, instruction, training
and supervision of all personnel is provided to
enable them to safely perform their work at
height.
 Scaffold platform must be clear from
obstruction, do not keep unnecessary
tools/material on platform & all tools/material
must be well arranged in a side.
 Use canvas basket & nylon rope for transfer
tools & light material.
 Use pulley system.
 Do not throw any tools/material from ground
to height and from height to ground.
 Use all mandatory PPEs all the time along with
full body harness.
 Peoples must use chip strap while work at
height.
 Provide barricade with warning signs around
scaffold.
 Before every use, ensure that scaffold is in
stable condition & all components are in place.
 Scaffold must be re-inspect after hit by any
moving equipment, alteration, or heavy winds.
 Erection, dismantling & modification is only
allowed to certified scaffolders under the

Use of scissor lift for work at height
supervision of certified supervisor.
 Unauthorized persons are strictly not allowed
to modify any component of scaffold.
 Unauthorized use  Scissor lift must be operated by certified
operator; he must hold valid TUV for the
specific equipment.
 Remove Ignition key (if fitted).Lock all
cabinets/covers when leaving the machine.
 Poor working environment  Scissor lift must be use in well ventilated area.
 Do not operate scissor lift in bad weather e.g.
heavy winds, heavy Rain, thunder and lightning
etc.
 Must be operated on firm level ground.
 Provide barricade with warning signs around
working area.
 Fire & explosion  Gas test results must be satisfactory.
 Remove all combustible/flammable material
from area, or cover with fire blanket.
 Never refuel the machine whilst it is being
operated or the engine is running.
 Always refuel the machine in a well ventilated
area.
 Ensure there are NO naked flames or ignition
sources.
 NEVER smoke during refueling.
 Avoid spilling fuel and immediately clean up
any spilled fuel.
 Collision with pedestrians, other vehicles  A trained flag man must be used while
or fixed objects while movement movement of scissor lift.
 Ensure all safety devices such as audible and
visual alarms are operating when the scissor lift
is in motion.
 Operator must follow flag man instructions.
 Overturn  Speed limit must not be exceeded.

 Falling from Platform.
 Tools & material may fall from platform
 Slipping on Platform Floor

 Clear all personnel from the area before raising,
lowering or slewing the Platform.
 Beware of over head utilities. Do not go very
close to the overhead utilities.
 Maintain mandatory minimum clearances from
overhead power lines.
 Look Up and Live(before and during operation).
 Remain within the confines of Platform. Never
sit, stand or climb on handrails.
 Wear full body harness & always ensure it is
attached to the specified anchor point.
 Never use ladders or scaffolding on the
Platform
 Never enter or exit the platform when
elevated.
 Scissor lift platform must be fitted with suitable
toe board.
 Barricade working area & place warning signs.
 Always maintain tools & material on platform.
 Ensure floor is kept clean, good condition and
intact.
 Avoid oil, grease and mud on boots on
platform.
 As practicable, maintain work platform/basket
clear of rubbish and tools.
 Always maintain a good foot and hand hold
when entering or exiting the platform.
 Keep personnel clear.
 Barricade off/designate work area.
 Never wear loose fitting clothing etc.
 Inspect cords & plugs for, cuts & damage.
 Ensure cords are protected from falling objects,
pinch points, or pedestrians or other traffic. Do
not pull cord around sharp edges or corners.
 Man lift must be operated by certified
operator; he must hold valid TUV for the

 Avoid congested work areas.
 Ensure sufficient clearance between the
platform and any overhead or other
obstructions.
 Keep clear of any obstructions that could
interfere with the raising or lowering of the
boom and watch for overhead obstructions.
 Maintain mandatory minimum clearances from
overhead power lines.
 Look Up and Live(before and during operation).
 Remain within the confines of Platform. Never
sit, stand or climb on handrails.
 Wear full body harness & always ensure it is
