AFFIDAVIT OF LOSS
I, REYNALD L. TORRES, of legal age, Filipino, and with residence and postal
address at Barangay Cabolo-An Sur, Oton, Iloilo, Philippines, after having been duly
sworn to in accordance with law depose and say:
1. I am a holder of Professional Identification Card for Professional Teacher
issued by Professional Regulation Commission;
2. I lost the said Identification Card on July 01, 2021, and that I exerted all my
efforts to locate the said card but I hardly remember where I placed it and it is
considered lost for all legal purposes;
3. The said card was not confiscated by any officer of the law by reason of any
violation of any law, ordinance, presidential decree, or rules and regulation of
the said Commission; and neither was it offered as collateral for any loan;
I am executing this affidavit to attest to the foregoing facts;
IN WITNESS WHEREOF, I have hereunto set my hand this 8 th of July 2021 in
