AFFIDAVIT OF LOSS

I, JASPER JOHN A. VELOSO, Filipino, of legal age, single and resident of 1027
Manggahan Compound, Barangay Sapa I, Rosario, Province of Cavite, after having been
sworn in accordance with law hereby depose and state that:

1. I applied for and was duly issued a LTO Driver’s License.

2. In the evening of November 1, 2020, while driving my motorcycle and doing

sideline as a delivery man for Lalamove, I inadvertently lost my driver’s license
somewhere along the way.

3. I searched diligently on every place where may I have lost the same but could not
find it such that I genuinely believe that it is now lost and beyond recovery.

4. My LTO Driver’s License was not confiscated by the lawful authorities nor was I
involved in any traffic accident.

5. I am executing this Affidavit to attest to the truth of the foregoing, to support my

application for the replacement of my lost LTO Driver’s License and for whatever
legal purpose it may serve.

IN WITNESS WHEREOF, I have hereunto affixed my signature hereunder this 9th
