An online registration form is a form or collection of personal information that an individual

must fill out before activating the usage of any software app online. Our book Open Mind, first
activity is to fill out some information about yourself and activate your barcode on line. I will
guide you through the whole process and your first homework will be filling out your personal
information.
I will send you a homework template in word file so you can update your information and the
resend it through the UTH Platform.
