1. Encoder will follow instructions in filling-out the template based from the Progress Monitoring on Self-Learning Modules (SLMs). 2. Provide data for both SLM Printing & Distribution for Centrally & Locally managed. Limit data entry to cells not shaded with colors, shaded cells are with formula, do not remove or replace the content. (Centrally are those modules provided by the Central Office while Locally are modules provided by the Region/Division. 4. Please ensure the value/data that will be encoded in each cell. Some of the cells contains formula that may change the template needed data. 5. Provide the date of data entry. 6. 7.
