Mandatory Health & Safety Training

Occupational Health & Safety

Awareness Training

Beginning July 1st, 2014 there are new training requirements under the Occupational Health and Safety Act
(OHSA). All Ontario employers must ensure that their employees complete an occupational health and safety
awareness training course to help protect them and keep them safe on the job. This training has been divided
into worker and supervisor courses.
This awareness training program will increase workers’ and supervisors’ knowledge of basic rights and
responsibilities under OHSA regulations and heighten awareness of basic workplace health and safety issues. It
will allow employees to support the Internal Responsibility System (IRS), which is based on the principle that
workplace parties themselves are in the best position to identify health and safety problems and develop
solutions. Employers, supervisors and workers share in the responsibility for occupational health and safety –
and your role and responsibilities are an important component of this awareness training course.

Please provide information for one of the following options to comply with OHSA standards set by the
Government of Ontario and return this form to Human Resources with your New Hire Paperwork.

I have completed OHSA Awareness Training with another employer and have attached document
confirming completion.
