Course Booking Form
How to book?
All bookings for the training courses must be made by fax or email. Please use the course booking form on the above page
to confirm your place.
Please note that once the KGS receives a completed booking form, your place on a course is confirmed. You are advised to
read and understand the terms and conditions outlined below.
Once we have received your booking we will send you a letter/email/fax of confirmation within 01 (one) working day.
Joining instructions including venue details will be issued approximately two weeks prior to the course.
Payment:
Payment for public course offering is due upon registration. You may pay by bank transfer/Pay order/Cheque/Cash as per
the details below. Course registration is not complete until payment has been received.
All fees quoted in data sheets, schedules, brochures or other documentation are exclusive of VAT at the standard rate. Fees for
scheduled courses include full course documentation, and refreshments as specified in the appropriate data sheet. Payment
in full must accompany this booking form. Please make cheques payable to (KGS QA).
Cancellation/Transfer Policy:
If you cancel your registration in writing 30 calendar days or more before the course, you will receive a 50% refund of the
course fee. No refunds will be provided for cancellations requested less than 30 calendar days of the course start date. You
may also contact us up to 21 calendar days prior to the course and make arrangements to transfer to another course. No
transfer request will be allowed less that 21 calendar days of the course start date.
Substitutes (swapping one person for another) will be accepted at any time.
In the unlikely event that KGS must cancel a course or change course location, liability is limited to the course fee. In such
circumstances, the customer has the option of a full refund or transfer to an alternative available course.
Travel Arrangements:
Lodging and travel cost are not included in the course fee. The individual attending the course must make their own travel
arrangements.
