Dear Sir,

We would like to offer our Department of Labor and Employment-accredited advanced OSH trainings
(per Republic Act No.11058 – An Act Strengthening Compliance with Occupational Safety and Health
Standards (OSHS) and Providing Penalties for Violations Thereof) below for your company.

Terms and conditions:

1. K.K. SAFETY TRAINING AND CONSULTANCY SERVICES shall provide the following:
a. Trainer/s who are experts in their respective fields of endeavor and duly accredited by DOLE
as OSH Practitioner/Consultant.
b. DOLE-recognized Training Certificates at the end of the course.
2. Cenapro Chemical Corporation shall provide the following:
a. Training room/venue that is conducive for learning.
b. LCD Projector/Screen
c. Refreshments and meals for the participants and Resource Persons.
d. Payment of the corresponding training fee stated above.
 e. Certificates of Appreciation for K.K. Safety Training and Consultancy Services for renewal
purposes.
f. Transportation for facilitator / Lecturer
g. Information on the training date 7 days prior to the conduct of training Information on the

3. Price quoted was based on 20 pax maximum participants. In case the actual attendees are below
the maximum number, the total price for the maximum participants shall be charged, in excess
of 20 pax additional per head will be charge to the client.
4. Full payment shall be made five (5) days before the start of the training either in cash or check
payable to K.K. Safety Training and Consultancy Services and deposit to BDO Savings
Account No.002930231889. In case of cancellation of the previously agreed training date, a
cancellation fee of PhP 10,000.00 will be charged to the client.
