WAIVER, RELEASE AND QUITCLAIM

2. I acknowledge and fully understand that the “Activities” involve inherent risks and
dangers that may cause serious injury and possible death to participants;
3. I declare that I am, or the minor I am representing is, fit to engage in the said
activities, without any life-threatening health conditions/issues, which is a
requirement by Viaje Royale Tours & Events Services Co. and its third-party
service/activity provider;
4. I assume all the risks in my/our participation in the said Activities;
5. I voluntarily waive any/all rights, actions, and demands whatsoever, including but not
limited to filing of any or all actions, suits or claims against Viaje Royale Tours &
Events Services Co., its board of directors, officers, employees, agents or
representatives, assigns or successor-in-interest, and hereby forever release and
discharge any or all of them from any cause of action, suits, sums of money, claims,
and demands whatsoever, in law or equity arising out of or in connection with the
said activities we/I availed of.

6. I hereby affirm that I have understood the contents of this instrument and I have
executed the same out of my own voluntary free will, without force, intimidation or
violence upon my person.
