ACTIVITY COMPLETION REPORT

I.TITLE: PROJECT SAGIP (Student Assistance through Guidance Intervention


Program)

II. LOCATION: Tabango Senior High School Campus, Tabango, Leyte

III. DATE: June 14 - 18, 2021

IV. INTRODUCTION
Despite the public health emergency brought about by this COVID 19 pandemic, the
department of education (DepEd) is committed to ensure unhampered delivery of basic
education services to its learners and community. In line with the constitutional mandate of the
state, “to establish, maintain and support a complete, adequate, and integrated system of
education relevant to the needs of the people, the country, and society-at-large,” pursuant to
section 2 (1), article iv of the 1987 constitution, as mandated in the republic act (ra) no. 10533
or the enhanced basic education curriculum 2018.
In accordance with DepEd Order No. 12, s. 2020, “Adoption of the Basic Education Learning
Continuity Plan (BE-LCP) for the school year 2020-2021 in the light of the COVID 19 Public
Health Emergency, to ensure the continued provision of learning opportunities to its learners,
while protecting the health and safety of both its personnel and learners. This can be done
through blended learning, distance learning, and home schooling.
As we battled against COVID 19 while continued on providing basic education services, it
cannot be denied that problems may arise in its implementation. Fifty-four (54) or 10.44% out
of five hundred seventeen (517) students have deficiencies or do not have outputs in their
subjects/classes.
To solve this problem, an intervention has to be made in order to save these students from
failing in subject areas which is relative to “no child left behind policy of 2010.” Thus, this
project SAGIP came into being.
In order to meet the objectives of this project, the said activity was proposed.

V. ACCOMPLISHMENT
The following were the accomplishments for the week:
1. The Learner Support Aide contacted some of the SARDOs through
messenger.
2. The Learner Support Aide visited a SARDO in their residence.

VI. PROBLEM/S MET


Some of the modules didn’t have names.

VII. RECOMMENDATION/SUGGESTION
Section Advisers should always remind their students to always write their
names on their modules to avoid blank grade sheets.

VIII. GOOD PRACTICES LEARNED


The value of cooperation and support to the learners helps the success of the
activity.

IX. SUSTAINABILITY PLAN


To sustain the plan:
Continuous participation and support of parents for all school activities.

X. REPLICATION
Parents together with the teachers must help one another for the success of
whatever plans the school may have for the better future of the learners
