1. PATIENT ADMISSION
Acceptance of new patients is a way of accepting the arrival of new patients in a room.
Example :
Admission  :  Good afternoon Mam. How can I help you? 
Patient  :  I want to check up. I have a headache. Where should I go? 
Admission  :  Here we have a GP (General Practitioners) and a specialist. Which one do you
want to choose, Mam?
Keluarga : Maybe you should go to a general practitioner.
Patient  :  Yes, I want to see a general practitioner.
Admission  :  As you can see, there is still a line. Please take queue first. We will call you later.
Please take a seat and wait patiently.
Admission  :  Queue No. 23. (Calling the patient). Excuse me Mam, have you been here before or
is this for the first time? 
Patient  :  This is the first time. 
Keluarga : what will we prepare for the registration process?
Admission  :  If this is the first time, may I borrow your Passport/ Identity Card, Mam? 
Patient  :  This is my ID. 
Admission  :  Thank you Mam. I borrow this ID to be copied. Here Mam I return your ID. Please
don’t lose it. After that, please fill in this form. 
Patient  :  Alright, here it is. 
Keluarga : Please speed up the registration, because my mother looks sick.
Admission  :  Thank you Mam for filling out the form. Please wait a moment, I will input the data
first. This is the evidence of the registration, please take it to the general polyclinic. Later you
will be called according to queue number. Please go to the 2nd  floor then turn right. 
Patient  : Alright, thank you.
Keluarga : Thankyou, good jo.

2. TRANSFER PATIENT
Patient transfer is the process of moving patients from one place of service to another
while remaining oriented to quality and patient safety
Example:
 Patient identity
 what is the patient's name?
 how old is the patient?
 what is the gender of the patient?
 Medical diagnosis
 What is the patient's diagnosis?
 General condition of the patient
 What is the patient's consciousness?
 How much is the patient's GCS
 Treating doctor
 The reason the patient was transferred
 The reason for being moved is because it requires further handling
3. Discharge Planning
Discharge planning is the process of preparing patients who are hospitalized to be able to
independently take care of themselves after treatment.
Example :
Nurse : "Good morning Mr. Wahyu, how are you today?"
Patient: "Good morning sir. Thank God it's getting better"
Anak : Did you take your medicine yet today dad?
Patient : I took the medicine this morning.
Anak :“Alhamdulillah, today there is good news for you. So today dad
allowed to go home.
Nurse : Yes right, but before going home I want to teach self-care at home”
ok sir here I will convey a few things, namely the first:
 You must adhere to the diet that has been set, namely low fat,
low in glucose, high in this to control blood fat, blood sugar and cholesterol. (brown rice,
avoid salty, offal, coconut milk dishes, and regular exercise)
 Signs of hypoglycemia (blood sugar levels drop) such as drowsiness, confusion, weakness,
cold sweats, vomiting starts, so you must immediately seek help to go to the health care
provider.
 Foot care and prevent new wounds such as not wearing narrow shoes must wear footwear,
avoid moist skin.
 Keep the wound clean and dry
 Avoid prolonged pressure on the injured leg
 Keep blood sugar control regularly
 Do not stop drug therapy without consulting a doctor
 Take medication regularly
Nurse : "how is it clear sir?"
Patient: "it's okay".
Nurse : "Try to do it again"
"Very good, Mr. Wahyu, I think you understand quite well what
delivered by the nurse. Thank you for your cooperation."
Anak : Calm down dad, I'll help dad at home later.
Patient: "yes sir, you're welcome"
