PERMIT TO WORK PROCEDURE

POLICY

It is the policy of PANAFRIC COMPANY Travaux to ensure that all civil work eg construction,
excavation, digging, shoring, cleaning, digging and maintenance works, servicing out of any sites are
controlled by written Work Permits issued from operators TPS
In case the permit is not in use, PANAFRIC COMPANY TRAVAUX will use the required work Permit
procedures as recommended.
Operator responsible person (Performing Authority) is requested to implement and follow up with the
Co-ordinator of the contractor (PANAFRIC COMPANY TRAVAUX)

PROCEDURE

1.0 SCOPE

The Procedure applies to all personnel working in PANAFRIC construction site.

2.0 PURPOSE

The purpose of this Procedure is to ensure that all work carried out on any company site is performed
consistent with the safety of personnel, the protection of the environment and equipment.

3.0 GENERAL DESCRIPTION

A Work Permit is a written method of ensuring that the work carried out is strictly controlled by
defined work practices and safety precautions designed to safeguard personnel and prevent damage to
the environment or equipment.

Permits to work procedures are necessary to help the civil work site supervisor to know the safety
precautions to be taken, and safety regulations to follow with each type of work.

3.1 Work Permit Request

A Work Permit Request must be raised to facilitate management’s overall planning of site engineering and
maintenance work. Where practical written requests for Work Permits should be logged with on site
supervisor at the installation at least 24 hours before the Work Permit is required. In case of an Entry into a
confined Space Permit, at least 48 hours prior notice should be given to facilitate planning and preparation
work. In agreeing the work content the Issuing Authority will specify on the Permit Request the Work
Permit(s)/ Certificate(s) which will be required and which must be issued before work commences.

3.2 General Work Permit ( i.e Cold Work)

A General work Permit ( Cold Work) is normally required for all work that is to be carried out on an
installation. However, the need for a General Work Permit will be at the discretion of the site supervisor,

and when a Permit is not considered to be required, the site supervisor should record this on the Permit
Request which must then be held in the Installation’s designated Permit Administration Point for the
duration of the work. The requester should also retain his copy of the request for the duration of the work.

3.3 Hot work Permit

No welding or flame cutting or any other work involving or giving rise to a source of ignition may be
carried on by any person than in accordance with the written instructions on a specific Hot Work Permit
applicable to that work.

A hot Work Permit coupled with a gas test of the area will be required for work on live instruments or
electrical equipment when such work defeats in build safety features, and the equipment is located in a
hazardous area.

Separate Work Permit will be required for different or unrelated but simultaneous work even when this
occurs within the same Area Authorities Zone.

3.4 Use of Equipment Requiring Hot Work Permits

Some examples of activities, which require a Hot Work permit are as follows:

a) Electrical welding and the use of naked flames


b) Use of hand and power tools which may be a source of ignition , e.g drills
c) The use of non certified electrical apparatus when it is to be used in a hazardous area
d) Opening live non-intrinsically safe electrical equipment when it is located in a hazardous area
e) Grinding

3.5 Entry into a Confined Space Permit

This Permit is MANDATORY before personnel are permitted to enter a confined space.

A Confined space can be defined as any work area with restricted entry and escape routes eg: Tanks,
Channel, Cellars or Roof voids

If more than two atmospheric tests are required during the life of the permit an Atmosphere Test Result
Sheet must be raised and its validity maintained for the duration of the programmed job.

A General Work Permit is not necessary required in association with an Entry Permit. The type of work
which may be done under the Entry Permit, without the issue of an accompanying General Work Permit, is
that which does not require detailed special precautions, eg visual inspections, minor cleaning activities.

3.6 Mechanical Isolation Certificate

This Certificate is normally used in association with an appropriate Work Permit.
It covers the mechanical

isolation of specified plant and should be used when necessary to amplify the precautions to be taken by
the Issuing Authority on behalf of the Performing Authority.

In certain circumstances it may be necessary to isolate the machine for an indefinite period, i.e while
awaiting long delivery spares or when use of the plant is prohibited for any reason ect in these cases a
‘Long Term’ Mechanical Isolation Certificate must be raised and controlled by the Issuing Authority.
No accompanying General or Hot Work Permits are required for these ‘Long Term’ Certificates

3.7 Electrical Permits & Certificates

The Electrical Permit system forms an integral part of the Work Permit system and comprises three Permits
and one Certificate.

One or more of these Permits may be necessary to allow work to be performed on electrical equipment. In
addition they must have an accompanying Hot Work or General Work Permit
