SCHEDULE OF CHECKING OF SCHOOL FORMS AND END OF SCHOOL YEAR RITES IN


PUBLIC ELEMENTARY SCHOOLS AND SECONDARY SCHOOLS

To : All Teachers

1. This is to inform all the teachers on the schedule of checking of all School Forms (SF-1-
10) and End of School Rites for the SY 2019-2020.
2. School Forms will be checked by the Division Checking Committee on the following
schedule and venue

March 18-20, 2020 Grade 6 & Honor Learners of Sto. Tomas ES
Grades 1-6
April 6-24, 2020 Kindergarten and Grades 1-5 Sto. Tomas Elementary School

3. Policy Guidelines on Awards and Recognition for the K to 12 Basic Education Program
(DepEd Order No. 36, s. 2016) shall be used in giving recognition to the learners. (Note:
The grades of the honor students must be in whole numbers as stated in DO no. 8 s.
2015.
4. Moving up Ceremony and Recognition Program for Elementary shall be done
simultaneously on March 30, 2020 except San Mariano Elementary School and
Penaranda North Central Schools.
5. Completion Program of Junior High Schools shall be done on March 31, 2020
6. Graduation Ceremony of Sto. Tomas Elementary School shall be scheduled on April 2,
2020 at 4:00 pm.

7. In all school forms, the grades of learners will be in whole numbers as stated in DepEd
Order No. 8, s. 2015

8. The new, standardized forms shall be used in all schools and ALS Community Learning
Centers for the school year in accordance to DepEd Order No. 58, s. 2017 (Adoption of
New Forms of Kindergarten, Senior High School, Alternative learning System, Health and
Nutrition and Permanent Records) downloadable at
http://www.deped.gov.ph/resources/downloads/schoolforms. Guidelines on the
adoption of the new forms (SFs-10) shall be strictly followed. In SF 5, the signatory for
the Division Representative shall be ASDS Ericson S. Sabacan, Ed.D. (for schools
Division Superintendent) and o be countersigned by the Monitoring Supervisors.
9. For the number of school days as referred to DepEd Order No. 007, s. 2019, the Total
Number of School Days is 203 inclusive of the five-day In-Service Training, three(3)
days for the Parents Teacher Conference (PTC) conducted within the school year, and
World Teachers’Day, less 2 Local Holidays (Sept. 2-Nueva Ecija Day and Oct 4-St.
Francis Feast). The total number of School Days is 201 as stated below.
