Geraga: This refers to your application for the initial accreditation of CMC Prime Builders and Trading Corp. mechanically operated (By Weight) asphalt batching plant located at Brgy. Malineng, Cuyapo, Nueva Ecija to supply bituminous hot mix to DPWH infrastructure projects. Please be informed that upon evaluation/assessment of the submitted documents by the BRS accreditors, said documents were found to be in accordance with Department Order No. 253, Series of 2003 "Guidelines on Accreditation of Portiand and Asphalt Cement Concrete Batching Plants”. However, it was observed by the BRS accreditors during the actual inspection/assessment of your batching plant that the procurement of basic asphalt testing equipment is still in progress, while the quality control of the materials and the bituminous hot mix are being undertaken either by government or other accredited private testing laboratories. In view hereof and pending compliance/procurement of the required asphalt testing ‘equipment, this Bureau hereby issues a special permit authorizing your company to supply bituminous hot mix to various DPWH infrastructure projects for a period of one (1) year effective March 12, 2020 to March 11, 2021. Moreover, as part of the monitoring (spot checking) activity of the Department as indicated in said Department Order under tem 3, Authorities and Responsibllties of the RS and Item 13, ‘Assessment by the BRS, an unannounced annual visit will be conducted to determine the continuous compliance of your batching plant. Please be reminded that fellure to maintain the requirements under Item 9, Terms and Conditions of the Certificate of the said D.0. may result to the cancellation or revocation of your permit issued by this Bureau.
