When I taught 3rd and 4th grade, I would use one Book Club or Literacy

Circle rubric per student over a course of four or five weeks to give me an
overall visual of their performance. I just checked whatever boxes I thought
suited their performance, and used a different color pen each time so I could
distinguish the days of my notes. (I also attached a slightly different rubric in
case you prefer to take grades of the individual meeting days.)

Then I’d send it home along with their self evaluations and any hard
copies of their completed work. You can drop by my store for a FREE copy of
the kid-friendly self evaluation or other Book Club related items. I’m working
on revising the items I used to teach Book Clubs right now, so if it’s not
available, please follow me for updates.

You’re welcomed to use this document however you want. If you post it
