I, the undersigned, hereby acknowledged the receipt of a copy of the AMA University Online
Handbook 2020.
I have read and understood the Student Handbook in its entirety, containing the institution’s
rules and regulations, policies and corresponding sanctions for the violations.
I have agreed to complete my exams within the given period of time of the subject course. Being
unable to comply with the final exam will means to forfeit my score.
I understood that taking with of all my subjects under Basic Education – Open Senior High
School will only last based on the 140 days residency.
I understood that retake is restricted of any assessments from any subjects involved.
It is my responsibility to communicate to my online instructors to make sure that I will comply in
a serious manner and complete the requirements.
I understood that any conflicts, discrepancies, or any suspected behaviour related to the exam
will be documented and a copy will be provided to the instructor
By signing this form, I confirm to abide by the institution’s policies and uphold the integrity of the
institution as its student. This shall not preclude AMA University Online from taking other course
of action in line with its concept of justice and righteousness warranted by the circumstance.
The issuance of this Handbook likewise does not preclude AMA University Online from making
unilateral amendments as deemed necessary.
