1. SCOPE OF WORK
1.1 General Description

• The general scope of works include supply & install of steel structure at Level 4
HVAC Room, Mixing Room 2 and 3, platform for Packing Room 1,2 and 3 and
also slab opening reinforcement works at Level 4 all in accordance to the
Approved Construction Drawings (2100a/P/PB/01_Rev 3 & 2100a/P/PB/02_Rev
3).

• Prior to the erection of the steel structural, ensure all the column area has been
erected first as per the approved construction drawings and the surface has
been cleaned free of dust.
• Column materials were arranged closed to design position.
• Column materials need to be cleaned, paint touching up and assembled before
erection.
• Columns should be arranged closed to their anchor bolt position.
• Attach driven rope to column.
• Check level and position of level nuts and anchor bolts with design.
• Check lifting weight and crane hook position with capacity of applied crane with
2.8MT.
• During erection work of column, lift the column and move slightly to design
position.

• Slightly down the column.


• Tighten anchor bolts nut after column in right position.
• The scissor lift will be use as work platform for controlling the crane hook and the
position must be at side of column to avoid from struck by the beam during lift up.
• Sliding plate will be put under the column before being lift up to reach a vertical
position.
• Once the column already at vertical position, 2 manpower need to hold the
column and controlling the movement of column to positioning on the bolt.
• Tightening the nut once the column already on the position.
• The chain block will hold the column until others column install.
• Type of anchor points shown in below. Check anchor points to ensure safe
anchor points for column after erection. These points will be drilled on casted
concrete structures such as existing concrete slab by drilling 4 nos of M20
Hilti/HAS/RE-500.
• Size of the base plate will be used was 350x350x20mm thickness S275 with
30mm of thickness non-shrinkage of grouting.

• Drilling the hole. Clean the hole immediately before setting the anchor. Remove
drilling dust and standing water from the base of the hole by blowing out well.
Ensure that the specified setting depth is marked on the anchor rod.
• After setting, adhesive mortar must fill the annular gap completely, right up to the
concrete surface. A load may be applied to the anchor only after the curing time
“tcure” has elapsed.

• The column that under the RC beam will install by using base plate hook as
shown in red marking that already install before while others will use lifting
frame.
• 2.8MT crane hook will be use as lifting device for lift up the column whereas will
attach to base plate hook and lifting frame to lift up.
• Standard equipment of beam clamp will be used with load and dimensions is
shown below.

• All the column and tie beam will lay horizontal at the point of installation for
easier during lift up.
• The lifting shackle will wrap at ¾ of the length from top of the beam as shown
in figure below.

• During erection work of beam, lift the tie beam and lifting slightly by using beam
clamp SWL 1T with shackle SWL 1T and hook by crane 2.8MT with sling belt
SWL 2T.
• Scissor lift will be use as work platform during positioning the I beam at the
column.
• The I beam will be installed to get the column safe for standing and lift by 2.8MT
crane hook that attach with base plate hook.
• 2 manpower on the scissor lift will hold the I beam and positioning at the column.
• The I beam will secure with bolt and nut once reach the positioning.
• Crane hook is only released when all connection bolts were tightened,
temporary bracings, bracing purlins, flange braces were installed adequately.
• All connection joints must be completed bolt tightening before releasing lifting
equipment.
• Steel erection installation is then completed. Similar installation procedures are
utilized for the next section of installation.
• The installation of the steel structure for I beam at Mixing Room 2 and 3 is
illustrated as follow:

• Concern to install all main column and tie beam to ensure the structure is
strong enough for positioning the equipment.
• The tie beam that obstructs during installing the equipment will install later
once the equipment already on positioning.
• The smaller tie beam as orange colour will be erect later just by using scissor
lift.
• The truss will be joint together with 3 section of the length on site by using bolt
and nuts.

• Slab opening layout of reinforcement for the Gridline A-C/14-17 are illustrated as

• Prior to the installation equipment and platform, ensure small openings in


existing slabs Level 4 area usually core-drilled to the required diameter which
is 250mm from Gridline A-C/15-17 while 400mm from Gridline B-C/14-16.
• Larger openings at 400mm diameter are cut with a circular saw or a concrete
chain saw with plunge cutting capabilities.
• Because a circular saw makes a longer cut on the top of slab than on the
bottom, small cores drilled at the corners can be used to help avoid over-cutting
opening when a circular saw is used.
• Cutting opening in existing slabs should be approached with caution and
avoided if possible.
• When cutting an opening in an existing slab, the effect on the structural integrity
of slab must be analysed.
• Add I-beam or tie beam reinforcement steel structure to the surface of the slab
using securely bolt and nut or post-installed anchors.
• The installation is fairly simple but because plates and through-bolts would
interfere with flooring surfaces, plates are normally installed on the bottom of
the slab using post-installed anchors.

• The lifting stand will install at 4th floor SBFI for lift up the reinforcement
457x191x67 KG/M UB through the holes.
• 3T chain block will be used as lifting device and controlling by experience
person.
• The communication will use walkie talkie during pull up.
• Before lift up the reinforcement 457x191x67 KG/M UB , the column 254x254x73
KG/M UC will lift up as per JDE-PT-MST009 steel structure erection.
• The bolt on RC beam will install by using M20 Hilti adhesive whereas required
for drill hole for put in the bolt.
• Vacuum need to put at the drill point for controlling the dust.
• During installing the steel structure manually, the reinforcement still hold by 3T
chain block that attach to lifting frame until the installation complete.

• Once the steel structure completed, the chain block will release slowly until the
reinforcement stand on the steel structure and secure by bolt and nut.
• The sling belt only can release after all the bolt and nut completely tighten.
• There are 2 tie beams need to install for complete the stand support.
• The 2 tie beam will install by using scissor lift from below and secure by bolt
and nut.

2 SAFETY AND HEALTH

2.1 Job Safety & Health Analysis


• A job Safety & Health Analysis shall be carried out for each work activity covered by
this Method Statement
• All control measures required for the activity will be clearly identified. A Job Safety &
Health Analysis is always required with a Method Statement

2.2 Information to Personnel

• A task briefing, LMRA based on the control measures detailed in the Job Safety &
Health Analysis is required for all work activity
• Site supervisor will responsible for task briefing assist by HSE Supervisor and
must record in Toolbox Talk form.
• Task briefing will be held daily every morning on 0830hrs.
• All team members is compulsory to attend

•Obtain emergency contact number i.e. police station, fire department, ambulance etc.

•Display the layout plan at prominent places and copy to sub-contractors.

•Plan and conduct evacuation drill and / or fire fighting training at least once a year.

•Monitor changes to layout and condition of equipment .

• Notification Of Emergency
The mobile phone is the primary means of emergency notification at the project site. This
system is intended for immediate transmission of specific information regarding an

emergency.

• Emergency respond plan


a. Emergency Response Plan (1) – Fire

In typical construction setting and site office, the diesel tanks and flammable
substance are the main risks factor in causing fire and explosion. Similar provision will
be made at flammable substance storage areas. Flammable substances will be stored
in suitably ventilated compound at designated locations except when in immediate
use. A fire exit plan should be prepared.
Site storage and offices areas should display appropriate fire advisory and warning
notices like “NO SMOKING”, “FIRE EXIT Route”, etc. The actions to be taken in the
contingency plan in event of fire include:

i. Sound the alarm


ii. Report fire location to the nearest personnel or ER Team and nearest
fire rescue department
iii. Turn off electricity and diesel powered equipment, if fire alarm activated
iv. Close all discharge outlets
v. Eliminate all sources of ignition
vi. Assemble all workers by ER Team
vii. Head count to be conducted by ER Team & Safety Personnel
viii. Prohibit entry to the site until safe to do so
ix. Record all event

b. Emergency Response Plan (2) – Structural Collapse and Technical Failure

In event of structural collapse or technical failure i.e. crane overturn, boring machine
collapse, scaffolding collapse, landslide, construction equipment collapse etc.; work
on site should be IMMEDIATELY STOPPED in an orderly manner. It may cause fatal
accident and injury incident. The contingency plan in should be taken include:

i. Stop work
ii. Sound the alarm
iii. Call Fire Bridged / Rescue Team / Ambulance
iv. If a fire threat, call fire service
v. If chemical spillage, cordon the area (Refer Chemical Spillage)
vi. Clear the area
vii. Assemble the workers
viii. Conduct head count
ix. Wait for emergency service arrival

c. Emergency Response Plan (3) – Fall / Rescue At Height

In event of fall or rescue at height to be performed, actions to be taken are listed below:

i. Dial 999 immediately and note time of fall.


ii. Report to the ER Team/Supervisor involved.
iii. Nearest observer to initiate verbal contact with the fall victim as soon
as possible to access consciousness, reassure and ask whether the
victim can “Self Rescue” provided appropriate equipment is available
and he have been trained to do so.
iv. Observer to decide whether self-rescue is possible considering other
factors as in if the equipment is visibly damaged.
v. If self-rescue is not possible – immediately set up the stand-by
vi. rescue equipment on hand.
vii. Rescue if safe to do so – if not safe, wait for emergency services to
arrive.
viii. Leave the rescue equipment in place as the emergency services may
be able to use some of the equipment and save time.
ix. The ER Team/Supervisor must be brief upon arrival. Types of items to
brief on are:
x. Type of emergency
xi. Location
xii. Degree of emergency
xiii. Extent of injury
xiv. Current situation
xv. Assist if required when asked by emergency services.

2.1 Permit Requirement


• This procedure is developed to control any activities considered being critical which
requires special safety precaution before actual work being carried out.
• Apart from the Sub-Contractor’s concerned, the Site Supervisor or even Safety Personnel
are allowed to act as originator of the Permit. I.e. General work permit, Hot works permit,
working at Height permit, Lifting Permit, etc.
• SHE Personnel will then continuously monitor the work in progress and if found any breach
of safety procedures, or any insufficient safety precautions, it may interrupt the work until
further compliance action or safety improvement being undertaken.
