Foreign Object Ingestion & In-Flight Shutdown (IFSD) of PW1100G-JM Engine

Applicability:
PW1100G-JM: PW1127G-JM, PW1127GA-JM

(A) Communication Overview:

The purpose of this communication is to inform the fleet that an A320neo PW1100G -JM
engine operator experienced foreign object ingestion and an In -Flight Shutdown (IFSD) on
May 27, 2021. Engine details: PW1127G-JM ESN 770765, TT/TC: 2,308 / 1,349. Pratt &
Whitney (P&W) is aware of the event and an investigation has been initiated.

(B) Content Details & Background:

During takeoff on a scheduled flight, the flight crew reported observing a fast -moving mammal
on the left side of the aircraft heading towards the runway followed by audible indications and
high vibration. After lift-off the engine experienced an un-commanded in-flight shutdown, the
pilot declared an emergency and performed an air turnback. The aircraft landed safely. The
Post Flight Report indicated multiple ECAM messages during the event, including ENG 1
STALL, ENG 1 OIL CHIP DETECTED and ENG 1 FAIL.

Post-event inspection of the engine showed one fan blade fractured above the platform and
damage to several other fan blades as well as damage to the fan exit guide vanes (FEGV’s).
Recovered mammal remains have been collected and sent to the Smithsonian f or analysis. The
engine is to be removed and sent to the MRO shop for investigation and repair.

(C) Conclusion:

The FAA has been notified of this event by P&W in accordance with FAR part 21.3.

This AOC will be updated as additional findings become available.
