Sir,

Greetings of health and safety!

Our office together with the Contact Tracing Team would like to inform your good office
that some of your employees were identified as Close Contacts of a Confirmed RT-PCR Positive
Patient last June 2, 2021. This letter signifies that the said close contacts from your office are
currently being monitored by the barangay nurse of their respective barangays.

In order to prevent the spread of the virus to your good office and their families, it is
recommended by the Surigao City Health Office that the following persons must undergo and
complete their 14-days Home Quarantine:

 Jerwin L. Odchimar – Level I (Quarantine Period: June 3-17, 2021)

 Noeberlito C. Espaldon Jr. – Level II (Quarantine Period: June 3-17, 2021)
 Kenneth S. Arnuza – Level II (Quarantine Period: June 6-20, 2021)

A certificate will be released by the barangay upon completion of the close contacts’
quarantine. The said certificate will serve as a proof of their clearance to resume work.

Whenever symptoms arise from the employees who are working in your office, it is
recommended by the City Health Office to immediately inform your Contact Tracing Team and
Barangay Nurse.

The foregoing is based on Republic Act 11332, the Mandatory Reporting of Notifiable
Disease and Health Affairs of Public Concerns, its implementing Rules and Regulations, relevant
issuance of Department of Health (DOH) and the IATF MEID.
