DUTY MEDICAL OFFICER (GDMO), under Health & Family Welfare Department, Government of West Bengal , you are being called
for Documents verification on the date, time & venue scheduled below. You will be allowed to appear at the interview on the
same day, if all the certificates are authentic & West Bengal Health Recruitment Board is satisfied that you fulfill the laid down
qualifications/ conditions etc. mentioned in the advertisement for the applied post.

Before coming to documents verification-cum-interview, you must ensure that you have fulfilled all the conditions of eligibility
criteria for the applied post as per Advertisement. You are requested to bring all the original documents along with two separate
sets of self attested photocopy (from originals) of the following in the same order. After verification, all the original documents
shall be returned back and self attested two sets of photocopies will be retained in our Office.
   List of Documents in 2 separate sets arranging in following order:

1. Copy of Interview Call Letter.

2. Copy of Application Form duly signed.
3. Colour passport size Photographs.
4. Caste certificate ( SC/ST/OBC) from competent Authority of West Bengal (if applicable).
5. Disability Certificate (40% and above) from appropriate Medical Board ;(if applicable).
. Photo ID Proof with Address like Passport / Voter / AADHAR/ Driving License etc.
7. Age Proof Certificate ;( Madhyamik or Equivalent Admit/Certificate).
. MBBS Degree Mark sheets & Certificates as per Advertisement.
9. Chance Certificate of MBBS Degree (if any)
10. Medical Council Registration Certificate.
11. Regular / Contractual W.B.Govt. Employee Certificate (if any)
12. Experience Certificate ( If any).

* Note : Whether in permanent service in Govt. of W.B. ,NOC from the Appointing Authority to be submitted in time of
interview.
Please note that you will not be allowed to appear at the interview if you have failed to produce any of the above required
certificates/ documents, in original.

Mobile Phone,Laptop,Scanner,Camera etc. are not allowed at the aforesaid Venue for interview.
No TA/ DA will be admissible.
