    By virtue of the power vested upon me by Republic Act No. 7160, otherwise
    known as the local government Code of 1919, particularly, Section 389 (b) (5) thereof,
    having met the qualifications set forth under Section 395 of the same Code. You are
    hereby appointed SK Secretary of Barangay Zone 1, subject to concurrence by the
    Sangguniang Barangay effective upon taking Oath of Office.

    As such, you are to perform the duties and functions of the office as provided by
    law.

    Please be guided accordingly.
