    Block 14 lot 13 Pamucutan Zamboanga City and Mr. Jayson G. Empase of legal age,
    representative of Green Leaf ZC. Lending Corporation.

    Do hereby agreed/ made an Agreement in this office in the presence of our Punong
    Barangay Hon. Roberto V. Carpio.

    That Mrs. Clarisa Rusapa Shall pay her Loan Amortization every 15 days starting 8 th of
    November and 28th November till Fully Paid . the payment shall be rendered to this office and
    Contacted Mr. Empase.

    In witness hereof were are here to affix our signature this 25 th day of October 2022 at
    barangay Pamucutan Zamboanga City.
