RE: WARNING LETTER FOR LEAVE WITHOUT INFORMATION

It has been noticed that you have taken leave since November 4 up to date without prior permission
from the company and/or without properly filing Leave of Absence. Such act results to wilful
disobedience and carelessness of duties, in your capacity as an Auditor.

Absenting yourself from roles without prior notice is a misbehaviour for which you are making yourself
responsible for the necessary action.

It has also come to our attention that you verbally resigned from your work.

In any case, you are hereby suggested to give a written explanation on your unofficial leave or written
letter of resignation within five (5) days after the receipt of this letter.

May I also remind you that employees leaving the Company should observe proper turnover for not less
than fifteen (15) working days.

Kindly treat this as an emergency.
