SUBJECT: Insubordination

This letter communicates the management’s decision concerning the recommendation for strict
disciplinary action due to your unsatisfactory job performance and unacceptable personal
conduct.

1. Sometime in May 2020, you were given a verbal Warning due to your lack of
performance and communication with your supervisor. These was due to the fact when
you are being called by your superior, you are either out of reach or not answering your
phone, despite of repeated tries;
2. A fortnight later on the 16th of May 2020, when no improvement was evident, you were
given a formal written warning as to the absolute necessity of improving your
performance, but to no avail;
3. That it was also indicated in the memo that you have received that you need to submit
show cause letter and come for a face to face meeting on May 18 at 10am,yet, no show
cause letter has been submitted to the company up to this date;
4. That on 1st of June 2020 --- a day before the scheduled meeting with the Company’s
President, you were also reprimanded by your superior verbally because of your
continued unacceptable personal conduct;
5. However, 2nd of June 2020 came and you didn’t show up in the scheduled meeting with
your superiors together with the company’s president who were expecting that all the
technical papers needed for the launching of MonteVille Residences will be finally
discussed;

These grossly inefficient job performances and your continuous unacceptable personal
conduct which resulted to the delay of the launching of our project are the main reasons for
 the recommendation for strict disciplinary action to submit a letter of explanation within a
maximum of 48 hours from receipt of this memo regarding this matter and kindly also
indicate the causes of action and remedies in order to avoid this kind of conflict again in the
future.

Moreover, you are required to submit on June 8, 2020 all the necessary materials that will
be needed for the launching of MonteVille Residences, particularly the Perspectives, Floor
Plans, Building Plans, detailed cost estimates, both actual and those which will be submitted
to Pag-ibig, and general specification each model unit.

I hope you will take this letter seriously and revert back to me PROMPTLY.
