MOTION TO ALLOW THE ACCUSED TO UNDERGO MEDICAL CHECK UP

AT NATIONAL CENTER FOR MENTAL HEALTH (NCMH) IN
MANDALUYONG CITY

ACCUSED, unto this Honorable Court, respectfully avers and moves

that:

1. Accused is presently detained at ______ Municipal Jail;

2. On 12 October 2020, the accused was brought by the jail warden of

_______ to NATIONAL CENTER FOR MENTAL HEALTH (NCMH) in
Mandaluyong City by virtue of a Court Order dated 21 September
2020;

3. On 04 June 2021, the undersigned received a text message

____________ informing her that the accused needs a follow up check
on 11 June 2021 at 9:00 o’clock in the morning;

4. Hence, the accused begs the compassion and understanding of the

Honorable Court and prays that a follow up check-up be conducted
on the accused at the National Center for Mental Health at
Mandaluyong City on 11 June 2021 at 9:00 o’clock in the morning.

PRAYER

WHEREFORE, in light of the foregoing, it is most respectfully prayed

that this Motion be GRANTED and that an Order be issued allowing the
accused to undergo medical check-up on 11 June 2021 at National Center
for Mental Health, Mandaluyong City at 9:00 o’clock in the morning.

Other reliefs just and equitable are likewise prayed for.
