JUDICIAL AFFIDAVIT

I, CYNTHIA E. OCHOSA, Filipino, of legal age, married, 60 years old,

with office at City Civil Registrar’s Office, Poblacion IV-A, City of Imus,
Cavite, for the purpose of proving the lack of valid marriage license of the
parties [Lishuang Zhan and Lianhua Xu], respectfully testifies by answering [under oath
and pain of perjury] the questions of petitioner’s lawyer [Miguelito Nazareno V.
Llantino] conducted at Imus Civil Registrar’s Office, as follows:

Q1 : Where do you work?

A1 : I am working at Civil Registry of Imus, Cavite, sir.

Q2 : How long have you been working at Civil Registry

of Imus, Cavite?
A2 : Since 1986, sir.

Q3 : At present, what is your official position?

A3 : I am the Registration Officer II, sir.

Q4 : What are some of your functions?

A4 : I do several administrative tasks that will provide a

 fast and accurate assessment and processing of various
applications for registration and issuance of marriage
license as well as birth, death and marriage certificates and
other certification, sir.

Q5 : Relative to this case and based on your records,

do you know if your office received a request from the
lawyer of the petitioner?
A5 : Yes, sir.

Q5 : If shown to you that lawyer’s request letter would

you be able to identify it?
A5 : Yes, sir.

Q6 : I am showing you the parties’ marriage certificate

marked as Exhibit “A”, which was attached to the
lawyer’s letter marked as Exhibit “B” requesting for
certified true copies of the marriage license no. 3795679
that your office issued on 18th December 1998 and the
application thereto. Please tell us what is the relation of
these documents?
A6 : This is the same marriage certificate and request
letter of the lawyer that our office received, sir.

Q7 : What did your office do with the request letter?

A7 : Our boss, Violeta P. Sanez, the Local Civil Registrar
of Imus, Cavite, instructed our staff to search our records,
sir.

Q8 : What was the result of the search?

A8 : Despite diligent search, our office found no record
of application for marriage license of the parties and
marriage license even if the entire records for 1998 are still
intact in our archives, sir.

Q9 : What did your office do next?

A9 : Our office prepared a Certification, sir.
 Q10 : If shown to you that official certification that your
office issued, would you be able to identify it?
A10 : Yes, sir.

Q11 : I am showing you an original copy of the Imus City

Civil Registrar’s Certification marked as Exhibit “C”
stating that its office has no record of application for
marriage license of Lianhua Xu and Lishuang Zhan xxx.
Hence, we cannot issue, as requested, a true copy It also
certified that records for 1998 are intact in its archive.
Kindly take a look and tell us what is the relation of this
document?
A11 : This is the same official certification that our office
has issued, sir.

Q13 : There are two [2] signatures on the Certification, do

you know whose signatures they belong?
A13 : On the left portion is the signature of Ariel Mari C.
Camerino as the one who verified and prepared
while the right was our boss, Violeta P. Sañez, sir.

Q14 : Why do you know the signatures?

A14 : Because I am familiar with their signatures, sir.

Q15 : Where is Violeta P. Sañez?

A15 : She resigned last December 16, 2020, sir.

Q16 : Do you confirm the fact that no marriage license

was issued to the parties based on your records?
A16 : Yes, sir.

I, MIGUELITO NAZARENO V. LLANTINO, Filipino, married, of legal

age, with law office at 209 Intramuros Corporate Plaza, Recoletos St.,
Intramuros, Manila, hereby attest that I [1] conducted the direct examination
of Cynthia E. Ochosa, Registration Officer V of Imus City Civil Registry; [2]
faithfully recorded or causes to be recorded the questions I asked in the
corresponding answers that the witness gave; and [3] no one coached or
assisted her.
