I am a student of the Mariano Marcos State University – Graduate School and I am

presently conducting a study titled “Senior High School Teachers’ Competencies in Content
Area Reading Instruction” as a requirement for the degree Masters of Arts in Education,
major in Language and Literature.
In this regard, I would like to ask permission from your office to allow me conduct an
online survey questionnaire to all teachers handling classes in Oral Communication in
Context, Earth and Life Science/ Earth Science, and General Mathematics, and administer a
document analysis on the Self-Learning Modules (SLMs) for the said Senior High School core
curriculum subjects. The respondents are requested to accomplish the online survey by
accessing this link: https://forms.gle/hhGESwUwxNMjXb3k8. Also, I would like to conduct an
online unstructured interview to at least three teachers who are teaching the
aforementioned core subjects in each of the public secondary schools of our division.

Rest assured that the conduct of this study will not interfere the lesson delivery of
teachers on their respective classes. Likewise, all responses shall be treated with utmost
confidentiality.
