AFFIDAVIT OF CANCELLATION OF BUSINESS NAME

REGISTRATION

I, LUZVIMINDA C. VERGEL, of legal age, Filipino, presently residing at

Barangay, Science City of Muñoz, Nueva Ecija, after having been sworn to in
accordance with law, hereby depose and state, THAT:

1. I am the owner of a business registered to the Department of Trade

and Industry as VERGEL VEGETABLE STALL with business
address at Barangay Poblacion West, Science City of Muñoz,
Nueva Ecija.

2. I am closing the said business due to establishment of another

entity under the style of FOOTWEAR .

3. I have no financial obligations to any supplier or creditor during
the course of my operation of the said business.

4. I am requesting the Depatrment of Trade and Industry to cause the

cancellation of my business name registration.

5. I am executing this affidavit for the purpose of testifying to the

truth of the facts herein contained and for the further purpose of
requesting the authorities concern to issue onein lieu thereof.
